Objective: To evaluate the clinical effectiveness of the porcelain prosthesis fused with different metal (PFM) with telephone return visit, clinical indicators and scanning electron microscopy (SEM) and to compare the surfacial appearance and oxidative corrosion of nickel-chromium, titanium alloy inner crown in order to discuss which was better to the periodonial health and to provid clinical reference evidence for the Prosthodontics. Preliminary study on the relationship between the discolored gingival after the non-noble metal ceramic repair and the corrosion property.Method: (1) According to a same doctor's treatment record, 336 cases of PFM patients were visited by telephone, whose repaired time was 1~14 years. Followed up observation was taken to acknowledged patients's satisfaction degree with the ceramic prosthesis, the using condition, discolored and healthy condition of gingival and the existing problems. (2) Grouped those who willing to the return visit based on the repair time, compared the gingival health, gingival color and marginal fit condition of 104 cases of Ni-Cr alloy ceramic crown after 1~14 year's restoration, among which 48 cases were male and 56 cases were female, aged 22~72 years old. (All cases had the treatment record of a same doctor and the hospital's denture processing records). SPSS13.0 was used for statistical analysis. (3) In order to acknowledge the surface appearance change of the PMF after used in the oral cavity,observed and compared 24 cases of removal chromium- nickel and 6 cases of titanium alloy inner crown by SEM, and then compared them with the new one. Explored the relationship between PFM surface appearance change and repair materials, repair time, and marginal discrepancy.Results: (1) Results of telephone return visit: The satisfaction degree of repair effect was as follow: 79.8% excellent, 9.8% good, 7.7% in general, 2.7% poor. 18.8 percent of the patients reflected problems after repairing. (2) Clinical Review: Investigated 104 cases of gingival conditions after 1~14 years of Ni-Cr alloy-porcelain repair, there were cases (34%) of gingival margin dyeing, cases (36.8%) of marginal unfit, cases (16%) of unhealthy gingival. There was no significant difference of the gingival status of Ni-Cr alloy-porcelain crown of different time (P>0.05). (3) SEM: The surface of new Ni-Cr porcelain crown was flat, smooth and uniform. There were also polishing traces and occasional micropores on the surface. The corrosion degree of Ni-Cr alloy-porcelain crowns after 2~5 years, 6~9 years, 10~12 years was similar. But the corrosion appearance change of the marginal unfit crowns was obvious than those fitting well. There was similar corrosion change between Nickel-chromium and titanium porcelain crown.Conclusions: (1) Regular telephone return visit was a good move to improve the satisfaction degree and the reputation of the hospital. It can not only discover problem in time but also apply theoretical basis for the clinical recover work. According to feedbacks from patients, the total satisfactory degree of the PFM was 89.6%, and had no significant relationship with the selected metal. (2) There was corrosion of Ni-Cr alloy-porcelain crown in the oral cavity, and no obvious relevance between the appearance change and the repair time, but the corrosion degree of the crown was correlation with the marginal discrepancy. And the corrosion degree would affect the gingival status. (3) There was no remarkable difference of the crown corrosion degree between Ti alloy and Ni-Cr alloy-porcelain in this study.
