Part I Advance investigation of inhibinB in the mechanism of polycystic ovarian syndromeobjective:To compared inhibin B levels in serum , follicular fluid and the expression of mRNA of INHB in follicular luteinized granulose cells from women with polycystic ovarian syndrome and controls, and to investigate the correlation between INHB and the anovulatary mechanism of PCOS.Methods:Serum and follicular fluid were obtained from 71 women on day 3 of menstrual cycle and oocyte retrieval. The granulosa cells were collected from follicular fluid. The INHB concentration was determined by ELISA, the mRNA expression of INHB were measured by RT-PCR.Results:The mRNA expression was significant higher in BMI-matched PCOS groups than non-PCOS groups(P<0.01), but the expression in obese women with PCOS was significant lower(P<0.01). Serum INHB concentration on day 3 of menstrual cycle of BMI-matched PCOS groups was higher than the controled group(P<0.05), and its levels in follicular fluid were also higher than the obese women either with PCOS or not(P<0.05). The serum and follicular fluid levels of INHB in PCOS and controlled group were negatively correlated with BMI. Conclusions: The expression of INHB mRNA in granulosa cells and the higher INHB concentration with PCOS women were demonstrated that INHB may have significant relationships with the PCOS, and it maybe plays a role in mechanism of the anovulation. Part II The levels of inhibin B in serum and follicular fluid with PCOS patients during IVF-ET period and the relationships of inhibin B levels with the IVF-ET outcomesObjective:To measure the levels of INHB in human serum and follicular fluid during controlled ovarian hyperstimulation in IVF-ET period, Then, investigate the relationships of INHB levels with the IVF-ET outcomes.Methods:The levels of INHB in human serum and follicular fluid was determined by ELISA in 71 PCOS cases in IVF-ET. In addition, the serum hormone levels on the day of transvaginal oocyte retrieval were assayed by chemiluminescent immunoassay in these patients.The correlation between the INHB levels and hormone levels,number of retrieved oocytes,fertilization rate , embryo implantation rate , number of quality embryos and clinical pregnancy rate were analyzed.Results:The follicular fluid levels of INHB in obese women either with PCOS or not was lower than the BMI-matched PCOS groups(P<0.05).In PCOS patients, the INHB levels of women who conceived were higher than those in women who did not conceive. The follicular fluid levels of INHB in PCOS and controlled group were negatively correlated with BMI. The correlation between the INHB levels and sex hormone ,the number of eggs , fertilized eggs and thickness of endometria was not found.Conclusions:The higher INHB concentration in women with PCOS suggest that INHB demonstrated significant relationships with the PCOS, and it maybe plays a role in mechanism of the anovulation. The lower level of INHB may impact on the numbers of retrieved oocytes and the IVF-ET outcomes.
