Objective To analyze the high risk behaviors of HIV/AIDS and its influencing factors among men who have sex with men (MSM ) in Fuzhou city , also to understand the status of HIV infection and Syphilis infection, and to explore the relationship between HIV infection, Syphilis infection with social and sexual partner characteristics of MSM, so as to provide evidence for the targeted intervention on HIV/AIDS/STD.Methods Between January 2008 and March 2008,215 MSM were sampled by convenience and snowball sampling, and had a face to face anonymous transect survey by well trained investigators with informed consent.Structured-questionnaire was used to collect the demographic data, knowledge about HIV/AIDS/STD, sexual behavioral information and sex orientation. 3-5ml venous blood samples were obtained to detect the related antigens or antibodies of HIV, Syphilis, hepatitis B virus and hepatitis C virus.χ2 test and multivariate stepwise logistic regression analysis were used for statistical analysis in this study.Result A total of 213 participants were enrolled. The median age of investigated MSM was 28 year-old, 56.6% of participants were self identified as homosexual orientation, 32.5% of them were bisexual orientation. The knowledge about HIV/AIDS/STD of MSM was high. Condom use during the last anal sex with men in the past 6 months was 74.7%. In the past 6 months,69.3% of them had 2 or more male sex partners,59.3% had unprotected anal intercourse with men,17.8% had commercial sex with men,and 23.5% had sex with women.Elder subjects and subjects working in the bathhouse had a higher ratio of unprotected anal intercourse;subjects with lower educational background had more sex partners and male-female sex.Subjects working in the park and boarding house had a higher ratio of commercial sex and male-female sex.Subjects with a bisexual sex orientation had a higher ratio of male-female sex.A total of 159 MSMs were tested. Syphilis was detected in 20(12.6%)subjects by TRUST and 41(25.8%) subjects by TPPA. HIV was detected in 3(1.89%) subjects;among them 1 subject had both syphilis and HIV. The accumulated infection rate of HIV infection and syphilis infection was 27.0%.HBsAg and HCV-Ab were positive in 20(12.58%) and 0% cases respectively. Multivariate logistic regression analysis showed that the aged (OR=2.5,95% CI 1.2-5.6) was associated with the risk of accumulated infection of HIV and syphilis.Conclusion Potential risk of HIV/STD epidemic threatens local MSM who spread the risk to general population.High risk behaviors of HIV exist in MSM population in the researched city.Interventions should be given according to the age, educational background,sex orientation,social and sexual partner characteristics etc.Continuing efforts to improve the coverage of HIV counseling & testing and strength the peer education should be maintained in the future prevention program for MSM. So intervention and control of the HIV/STD prevalence in MSM and preventing the infection from MSM to common people is urgently.
