Objectives: The thesis is to establish rat model of MODS and investigate dysfunction and the changes of structure of beta cell of islet in Rats with MODS,the thesis is also to investigate the possible effects of anisodamine in rats with MODS.Methods: Forty-five healthy SD rats were randomly divided into three groups: normal control group(group A), MODS model group (group B)and anisodamine group (group C). The rats of group B and group C were made by left legs fracture as'the first hit'and then with Escherichia coli peritoneal injection as'the second hit'. The rats of group C received anisodamine peritoneal injection. The changes of pancreatic, hepatic, pulmonary morphological damage were observed with light microscope in each group.The changes of pancreatic ultra-structure damage were observed with electron microscope in each group. Try to measure the changes of FGLU,ALT,AST,CREA,BUN,ALB,PLT,lactic acid,ATPase, blood glucose 30 min after the glucose load, the amount of fasting insulin and the amount of insulin 30 min after the glucose load. Then the Indexes of HOMA-IR, HOMA-β, andΔINS30/ΔGLU30 have been analyzed.Results: 1. The chief pathological changes of lung, hepar, pancreas were apoptosis,cell degeneration,and the imbibition of inflammatory cell in rats with MODS. 2.The CREA, BUN, ALT, AST, blood glucose 30 min after the glucose load, FGLU, the indexes of HOMA—IR, and lactic actic were obviously increased in group B compared to group A(P<0.05). While the amount of insulin 30 min after the glucose load, PLT, the indexes of HOMA—β,ΔINS30/ΔGLU30, ALB, Na+K+ -ATPase and Ca2+Mg2+-ATPase were obviously decreased in group B compared to group A(P<0.05). Between the group A and B, the difference of the amount of fasting insulin was not significant(P>0.05). The indexes of HOMA-IR was obviously decreased in group C compared to group B(P<0.05), while Na+K+ -ATPase and Ca2+Mg2+-ATPase were increased in group C compared to group B(P<0.05), the changes would be made by anisodamine.Conclusions: The rat model of MODS can be successfully reproduced by two-hit method.The rats in MODS model group had disorde of glycometabolism.Both insulin resistance and pancreaticβ-cell dysfunction,the two pathogenic mechanisms made the disorde. The results of light microscope and electron microscope showed islet cell, pancreatic acinar cell,and centroacinar cell had typical apoptotic changes. And pathological changes consistent with acute pancreatitis. Rats with MODS exist energy metabolism dysfunction, it probably cause beta cell of islet damage. Anisodamine showed its beneficial effects on disorder of energy metabolism and insulin resistance in rats with MODS.
