Object:To evaluate the age factor in the role of treating the developmental dislocation of hips(DDH) by Salter osteotomy through studing the result of the dislocation of hips by Salter osteotomy after a long-term follow up.Patients and method:The clinical and imaging data of 65 DDH patients under Salter osteotomy were retrospected from August 2002 to Janulary 2008,Among them,9 were boys and 56 cases were girls.According to the age level,all patients were divided into two treatment groups with the principle of matching.Group A were patients who were accepted Salter osteotomy at the age of less than 4-year-old while Group B were at the age of more than or equal to 4-year-old.The acetabular index,CE angle,acetabular depth/diameter index, the function of hip by Mckay,hip X-ray by Severin standard were recorded before operation and postoperative and one year after operation.All these data,including the average acetabular index,CE angle,AI(D/W) index,Mckay score and the postoperative X-ray Severin score,were analyzed by the the General Linear Model of SPSS statistics software. Results:The average acetabular index(AI) of group A in pre-operation is 37.8°while AI in postoperation is 18.9°and 16.8°in one year after operation;the average AI of the group B in pre-operation is 38.2°while AI in postoperation is 23.1°and 21.6°in one year after operation,the two groups are statistically analyzed,the difference is significant(P<0.05). The average CE angle of group A is 37.2°in postoperation and 40.3°in one year after operation while group B is 34.8°in postoperation and 36.5°in one year after operation, the two groups are statistically analyzed,the difference is significant(P<0.05).The average AI(D/W) of group A is 30.9 in postoperation and 33.7 in one year after operation while group B is 31.1 in postoperation and 32.6 in one year after operation, the two groups are statistically analyzed,the difference is significant(P<0.05);the Mckay score and X-ray Severin score in six months after operation and one year after operation are both good,but between A and B are statiscally analyzed,the significance is obvious(P<0.05).Conclusion:There is a better prognosis and more conducive to the hip form and plastic in the children with DDH less than 4-year old if it is possible to treat DDH with the Salter osteotomy.
