The Clinical Observation Of The Treatment On Knee Osteoarthritis By Articular Injecting Of HA, Fumigating And Washing Of Chinese Herbs And Functional Exercise

Objective:To evaluate the clinical effect of the treatment on knee osteoarthritis by articular injecting of HA,fumigating and washing of Chinese herbs and functional exercise.Method:80 cases of knee osteoarthritis from Hubei TCM Hospital were randomly divided into 2 groups,the TG and the CG,each group has 40 cases.There are 45 females and 35 males.The eldest is 69 years old,and the youngest is 40 years old.11 of them have pain in both knees,69 of them have pain only in one knee.In the TG,18 of them are males,22 of them are females,age from 40 to 68 years old,average 55。1 years old;In the CG,17 of them are males,23 of them are females,age from 41 to 69 years old,average 54.4 years old.Clincial symptom mainly manifest arthralgia of knee,rest pain,after stair activity or walking for a long time,and all the patients to various extends have joint motion dysfunction,arthrocele,hydrops articuli,and joint thickening malformation.The affected knee has part of tenderness,patella grind test(+).X-ray performance shows the space of knee is narrow,condyles of femur or tibial plateau has some hyperostosis,spinous process of tibia sharped,articular surface is rough.Before the treatment all the patients'diagnosis of X-ray performance is classify by the Kellgren-Lawrence diagnostic code.In the TG,stageⅠ14 cases,stageⅡ20 cases,stageⅢ6 cases;In the CG,stageⅠ15 cases,stageⅡ18 cases,stageⅢ7 cases(Our test don't observe stageⅣpatients).In the course of disease,the longest is 4 years,and the shortest is 6 months,average 18 years.Test by Pearson Chi-Square,we found two groups of patients have no significant differenece in gender,age,X-ray performance.The diagnosis standard is according to the《Diagnosis principle of K0A》and《Clinical research instruction of the traditional Chinese medicine》.According to the Lysholm's principle about knee's function before and after the 5 weeks'treatment,and the standard of rheumatism involving bone in《Standard TCM of the People's Republic of China》,we summary the data and use SPSS13.0 software to analysis the date.Results:According to the Lysholm's principle about knee's function before and after the 5 weeks' treatment,we classify the difference exceed 30 is Excellent;the difference between 11-29 is Good;the difference between 6-10 is Fair;the difference less than 5 is Poor.In the TG,Excellent 19 cases,Good 15 cases,Fair 6 cases,Poor 0 cases,finess rate is 85%;In the CG,Excellent 11 cases,Good 14 cases,Fair 15 cases,Poor 0 cases,finess rate is 62.5%we use Pearson Chi-Square to analysis the date by SPSS13.0 software and we found there are significant difference between the TG and the CG.According to the standard of rheumatism involving bone in《Standard TCM of the People's Republic of China》,and use the Pearson Chi-Square to analysis the date by SPSS13.0 software,we also found there are significant difference between the TG and the CG.All the 80 cases were followed up for 3～6 months.And all patients have greatly improved in the function of knee,and none have side effect in the treatment.And the TG is much better than the CG in improving the function of knee and easing the symptom of knee. There are significant differenece between the two groups(P＜0.05).Conclusion:From the clinical observation,we can see that the combined treatment of articular injecting,of HA,fumigating and washing of Chinese herbs and functional exercise is more effective than only treat by articular injecting of HA and function exercise.With the fumigating and washing of Chinese herbs,the function of knee can improve more quickly.Generally, the treatment on knee osteoarthritis by articular injecting of HA,fumigating and washing of Chinese herbs and functional exercise has satisfactory effect on curing osteoarthritis of knee joints in early and intermediate stage,and it is worth to be popularized.
