Objective:Lower extremity arterial disease incidence is rising,non-invasive imaging has been applicated in the diagnosing lower extremity arterial disease.We investigated the value of the color Doppler ultrasound(CDU),computed tomography angiography(CTA) and magnetic resonance angiography(MRA) in diagnosis of lower extremity arterial disease.According to clinical diagnosis and treatment,we use the operational research theory of mathematical optimization model and propose the optimal imaging method.Materials and methods:We chosen the popular image optimization equipments, including the 7~11MHz color Doppler ultrasound,16-slice CT,1.5T MR.Analysing the images of ower extremity arterial disease with surgerical and pathological proved. Thirty-five were operated by CDU and sixty cases by CTA.The materials of MRA were selected from the literature search.To evaluated the extension,stenosis, calcification and occlusion in lower extremity arterial disease.The final evaluation factors in the diagnosis of vascular disease that may affect the relevant parameters were selected.The experienced radiologists and vascular surgeons estimated 37 factors in diagnostic,examine and clinical aspects respectively. We choose the operations research and Laplace rule to calculated the score,obtain optimal imaging method.Result:①CDU display of two-dimensional imaging of the lesions showed lesions of vascular wall and plaque with low,strong or mixed echo.Color doppler flow imaging (CDFI) showing color flow proximal stenosis weak image,colorful mosaic-like and luminal occlusion paragraph.②CTA can demonstrate a clear interest blood tree. Three-dimensional reconstruction imaging can show multi-angle and direction of wall calcification,irregular luminal stenosis,partial occlusion of blood vessels and collateral circulation.③Comparison of CDU and CTA examination,results showed that CDU and CTA have a statistically significant difference(P<0.05) in finding iliac artery lesions.There is no significant difference(P>0.05) in above and below knee popliteal arterial lesions.CTA has a higher correct diagnosis than the CDU in the superficial femoral artery severe stenosis or occlusion.④MRA is synthesis image of lower extremity arterial stenosis and determine the extent of vascular occlusion by software applications.⑤In all the factors that may affect the diagnosis and treatment in lower extremity arterial disease we select twenty-six diagnostic factors,six examination factors and five clinical factors.⑥To 37 factors,the results showed CDU got 1 score in 10 factors,CTA got 1 score in 27 factors,MRA got 1 score in 13 factors.⑦To solve optimization problems in the disciplines branch,select mathematical model to optimize the calculated score.To evaluate diagnostic factors, CTA score of 0.923 for the best.To evaluate examination factors,CTA score of 0.875 for the best.To Clinical factors,CDU score of 0.950 for the best.To consider all the three factors,CTA score of 0.766 for the best.Conclusion:CDU,CTA and MRA in the diagnosis of lower extremity arterial disease have advantages respectively in clinical diagnosis and therapy.The Laplace rule of the operations research could be applied to evaluate the optimal imaging method.CTA is the optimal method of non-invasive diagnostic imaging methods in lower extremity arterial disease.
