Research Purpose Placenta increta refers to the placental villus abnormal attachment caused by the decidual basement of the dysplasia,is a serious obstetrics and gynecology emergency,but also is an important reason to cause maternal postpartum hemorrhage.In recent years,with the cesarean section rate gradually increased,the number of placenta increta patients also increased significantly.Ultrasound is the primary diagnostic method.Magnetic resonance imaging(Magnetic resonance imaging MRI)is also an adequate diagnosis of placenta increta.This paper aims to compare the value of color Doppler ultrasonography with nuclear magnetic resonance imaging,and the combined use of both in the diagnosis of placenta increta.Materials and Methods In this study,we retrospectively analyzed 62 patients with clinically suspected Placenta increta from July 2013 to May 2015,who involved placenta previa,,repeated cesarean section history and repeated painless vaginal bleeding in late pregnancy.All of the patients who were after 28 weeks pregnancy took color Doppler ultrasound examination and nuclear magnetic resonance imaging examination.The span of the two examinations is ? five days.Eventually found that Placenta increta confirmed by surgery or pathology diagnosis or clinical analysis determined comprehensively.In this study,the use of color ultrasound diagnostic apparatus is E8,GEV730,and SEQUIA-512.The use of nuclear magnetic resonance imaging is Siemens Symophony 1.5T superconducting nuclear magnetic resonance scanner,and owned by Siemens LEONARDO image processing workstation.SPSS 22.0 software collected the data,and the chi-square test was carried out.In the study,we used the Mc Nemar test and Kappa coefficient test.Among them,Mc Nemar test,? = 0.05,if p ? 0.05 the difference was statistically significant;p> 0.05 the difference did not have statistical significance.The value obtained by the Kappa coefficient test can be divided into six sections,which is the degree of consistency.If k< 0,it means that the degree of consistency is very poor;if k = 0.0 ~ 0.2,it means mild weak degree of consistency;if k = 0.21 ~ 0.40,it means weak degree of consistency;if k = 0.41 ~ 0.60,it means a moderate degree of consistency;if k = 0.61~ 0.80,it represents a high degree of consistency;if k = 0.81 ~ 1.00,it means a strong degree of consistency.Research Result1.Diagnosed by Color Doppler ultrasonography,there were suspicious placenta increta 49 people.The final diagnosis of placenta increta was 46 people.There were 13 people who were not diagnosed placenta increta by color Doppler ultrasound.The final diagnosis of non-placenta increta was 9 people.We can conclude that the sensitivity of color Doppler ultrasound examination is92.00%,the specificity is 75.00%,and the accuracy rate is 88.70%.Hence the color Doppler ultrasound examination has a high diagnostic accuracy.2.Diagnosed by nuclear magnetic,there were suspicious 50 placenta increta people.The final diagnosis of placenta increta was 46 people.There were 12 people who were not diagnosed placenta increta by nuclear magnetic.The last number of non-placenta increta was 8.The sensitivity of nuclear magnetic is92.0%,the specificity is 66.67%,and the accuracy rate of is 87.09%.We can conclude that nuclear magnetic examination also has a high diagnostic accuracy.3.Color Doppler ultrasound and nuclear magnetic joint diagnosis of suspicious placenta increta were 45 people.The final diagnosis of placenta increta was 43 people.The two combined diagnosis does not consider placenta increta were 8 people.The final diagnosis of non-placenta increta was 7 people.It can be drawn the sensitivity of the combined examinations is 97.73%,the specificity is 77.78%,and the accuracy rate is 94.33%.The two joint examinations can significantly improve the sensitivity,specificity and accuracy,and can reduce the false positive and false negative rate.Analysis Conclusion1.Color Doppler ultrasonography has a high diagnostic accuracy for placenta increta,therefore the preferred diagnostic method.MRI as an auxiliary imaging diagnosis,its interpretation of the results can be combined with the results of the ultrasound.2.Color Doppler ultrasonography and nuclear magnetic both have the good diagnostic value for placenta increta.The two joint examinations can significantly improve the diagnostic value,effectively reduce the misdiagnosis rate and missed rate.
