Objectives:1. To detect the expression of MIF, the positive rate of Hp infection and expression of Hp cagA in normal gastric mucosa, chronic superficial gastritis, chronic atrophy gastritis, Intestinal metaplasia, Atypical hyperplasia, gastric cancer and gastric ulcer patients and evaluate its significance of early diagnosis of gastric cancer.2. Explore the role of Hp cagA and MIF in the development of Hp-related disease and the relevance of MIF and Hp through detecting the change of MIF expression after eradication of Hp, explore the correlation between Hp-related disease and Hp cag A.3. Search the performance of different detect methods of Hp.Methods:The biopsy tissue of gastric mucosa was gathered through gastro endoscope 81 patients with chronic superficial gastritis,57 patients with chronic atrophy gastritis, 28 patients with Intestinal metaplasia, 28 patients with Atypical hyperplasia, 50 patients with gastric cancer, 50 patients with gastric ulcer and 25 Patients with normal gastric mucosa were included , Hp was examined by urease test, Warthin-Starry, 14C breath test, immunoblotting and immunohistochemical method, MIF protein was examined by immunohistochemical SP staining method, MIF mRNA was examined by hybridization in situ, Hp cag A was examined by PCR ,Then Hp eradication was conducted on Hp infected chronic superficial gastritis, atrophic gastritis and gastric ulcer patients, Hp and MIF was recheck after eradication, compare the difference of MIF expression between Hp infected patients and Hp eradicated patients. Analyses effect of Hp infection and Hp cag A on MIF expression of relevance of Hp cag A and Hp-related disease.Results:1. There were 215 cases of Hp positive in 293 cases(73.4%), Hp in chronic atrophy gastritis, Intestinal metaplasia, Atypical hyperplasia, gastric cancer and gastriculcer were significant higher than chronic superficial gastritis(P<0.05),but there no significant difference between each group of chronic atrophy gastritis, Intestinal metaplasia, Atypical hyperplasia, gastric cancer and gastric ulcer were significant higher than chronic superficial gastritis (P >0.05).2. The expression of MIF is low in normal gastric mucosa without Hp infection ( 8%), but significantly higher in abnormal gastric mucosa (P < 0. 01), The expression of MIF increased with the serious of pathological changes , there is a significant difference between superficial gastritis and atrophic gastritis(P< 0.01), between pre-cancerous (CAG, IM, ATY) and gastric cancer(P < 0. 01),there was a high expression of MIF in many infiltrative inflammatory cell and gastric mucosa epithelial in gastric ulcer.3. The expression of MIF of Hp positive was higher than Hp negative gastric mucosa in chronic superficial gastritis (P < 0. 05)and atrophic gastritis(P < 0. 05), there was no significant difference between Hp positive and Hp negative gastric mucosa in gastric ulcer, Intestinal metaplasia, Atypical hyperplasia and gastric cancer(P > 0. 05).4. The expression of MIF was noticeable decreased after Hp eradication than before(P < 0. 05) , however there no significant change with those patients who Hp was still positive5. The positive rate of cagA is 81.4% in Hp positive gastric mucosa, detection in serum showed the main anti-Hp body was type I (84.2 % ),there was no significant different cagA and MIF and the serious of pathological degree.6. Investigation showed that the methods of Western blot and 14C breath test were sensitive than others of 5 kinds of methods, the sensitivity of immunohistochemical method was lowest, rapid urease test was simple, easy, cheap and warrthin- starry stain was accurate with background of clear.Conclusions:Hp potentially induces the expression of MIF on gastric mucosa and correlation with development from gastritis to gastric cancer. The type I is the main type of Hp in our erea and there was no significant relevance in cagA and the serious of pathological changes and expression of MIF. Eradication of Hp could cut down the abnormally high expression of MIF and slow down the formation and development of gastric carcinoma. Different method should be choice according different purpose...
