【Objective】To explore the levels of IL-6,IL-8,LTB4 in sputum and serum,which were in the patients with AECOPD after different treatment,and to obsreve the changes of these cytokines and the relation whith the FEV1 and FVC among these patients before and after treatment,in order to evaluate the role of inflammation factors,such as IL-6, IL-8,LTB4 during the inflammation of the airway,the level of the airflow obstruction and the clincial efficacies of corticosteroid treatment in COPD.【Method】A total of 33 cases of AECOPD patients were selected from June 2008 to March 2009 and randomly departed to A and B.Pulmonary function was measured and specimens of sputum and blood were collected during the admission and discharge. Concentrations of IL-6,IL-8 and LTB4 were analyzed using enzyme-linked immunosorbent assays(ELISA),which were in sputum and blood.The smears of sputum were stained with Wright's method,while sputum samples were tested for cell count and magnified 100 times by optical microscope.【Result】1.Neutrophils was found increasing in the sputum of patients with AECOPD,the results were as follow:(84.33±7.58)%in Group A and(80.14±5.74)%in Group B.While the eosinophile granulocytes were found in the minority,with (1.22±0.45)%in Group A and(1.12±0.45)%in Group B.After treatment,the amount of neutrophils in Group A decreased significally than those in Group B(t =4.458,P=0.001).Moreover,the Neu%in sputum was significantly correlate with VAS,IL-6,IL-8,LTB4 and had an inverse correlations with FEV1/FVC before the treatment.While the change of Neu%in sputum had a significant correlation with IL-6,IL-8,LTB4 levels and the perception of dyspnea after treatment.2.Before the treatment,the levels of IL-6,IL-8,LTB4 in serum and sputum were significantly higher than those without treated.The levels of IL-8 were correlate with LTB4 and IL-6 in sputum.While the levels of IL-6 were correlate with IL-8 in serum.After the treatment,The chang of IL-8 was correlate with LTB4 and IL-6 in sputum.While the chang of IL-6 was correlate with IL-8 in serum.3.Dncreasing of FEV1,FEV1%,FEV1/FVC in all patients will coincident with increase of the suptum/serum level of IL-6,IL-8,LTB4 before the treatment.They had an inverse correlations.After the treatment,the suptum levels of IL-8,LTB4 had the inverse correlations with FEV1,FEV1%of all patients.And the suptum levels of IL-6 had an inverse correlation with FEV1/FVC of all patients.4.Before the treatment,the dyspnea in all patients had inverse correlations with the sputum levels of IL-8,LTB4,FEV1,FEV1%and FEV1/FVC.There was a significant correlation with the perception of dyspnea and the changes of IL-6,IL-8,LTB4 in sputum before and after the treatment.Where the correlation with IL-8 were remarkable,the results were(r=0.360,P<0.05),(r=0.518,P<0.05),(r=0.349,P<0.05) respectively.Moreover,the perception of dyspnea was correlated with FEV1%significantly.5.There was a significantly linear correlation between the serum levels of CRP and IL-6(r=0.35,P<0.05),since the serum level of CRP increased simultaneously with the levels of IL-6.The levels of CRP were correlated significantly with WBC in the blood.6.Compared with those patients without receiving the corticosteroid treatment,those receiving the corticosteroid treatment resulting in the shortening of the hospitalization and the perception of dyspnea showed a significantly differences. Moreover,the levels of IL-6,IL-8,LTB4 in serum and sputum decreased significally,while the FEV1%and FEV1/FVC showed an remakable improvement. 【Conclusion】In those AECOPD patients,the levels of the inflammatory factor in airways and serum showed the significant alterations.All these changes had the correlation with the pulmonary function and the presences of clinical symptoms.It is suggested that the early corticosteroid treatment can relieve the inflammation of airway,improve the pulmonary function and alleviate the discomfort of the patients.
