Objective The goal of the research was to investigate the application of the computed tomography perfusion imaging in the cerebral vasospasm caused by intracranial subarachnoid hemorrage. CTP can provide the degree, extent and quantity of the ischemia caused by the cerebral vasospasm. CTP can play more important role in the diagnose and treatment in CVS after the rupture of intracranial aneurysms.Methods 30 patients with intracranial aneurysms who received endovascular treatment were selected in this study from August 2007 to December 2008. There were 17 cases of anterior communicating aneurysms, 11 cases of posterior communicating aneurysms and 2 cases of middle cerebral artery aneurysms. All patients received DSA and endovascular therapy after rupture of intracranial aneurysms immediately. The CTP and TCD was adopt at 3,7,10,14 days after operation. CTA,DSA were used to diagnose the CVS in different times.Resultes 10 cases were diagnosed the CVS in our study. IN no CVS group, CBF was significantly decreased at 14th day, the MTT was significantly prolong at 10th and 14th day . IN CVS group, the MTT was always significantly found prolong first, then the CBF could be found decreased. The CBV was not found decreased in every cases. It always decreased up to 14th day and the persistence time was comparatively long. At the same time , TCD only be found increase significantly at 14th day . Conclusions The MTT is the sensitive index to judge the CVS after intracranial subarachnoid hemorrage. CBF can exactly reflect perfusion condition of brain tissues and has comparative stability and sensibility. The significantly decrease of CBV is the comparative precise index to predict the irreversibility infract. Analyzing the CBV and CBF simultaneously can provide the important value to predict the degree, extent and quantity of the ischemic region .TCD could be used to judge the clinic prognosis, but the sensibility of the TCD is inferior to the MTT and CBF.
