The Research On The Characteristic Of Traditional Chinese Medical Syndrome In Sepsis

This thesis consists of two sections:literature review,and clinic research。literature review discusses the two aspects:traditional Chinese medicine for Sepsis,and western medicine for sepsis。In the aspects of western medicine,we summarize about the research progress of the cause and treatment of Sepsis in recent years。The TCM part is the discussion the research result of TCM for Sepsis。Its main content includs:1.the traditional Chinese medicine' s view of Sepsis,including use the way of TCM to diagnosis and treatment。2.the view of modernization of TCM for Sepsis,including applying with the explanation of herbs in western medicine to settle some problems, applying with the pattern identification theory of Chinese medicine in some phrase of Sepsis,researching the systemic pattern identification of Chinese medicine in the overall process of Sepsis。The part of clinical research can be detailed from the following four parts:ObjectiveThrough clinical features of 86 cases of sepsis patients,which from emergency section,ICU section,respiratory section,kidney section and other related sections in Guangdong provincial hospital of TCM in February 2007-March 2009 in year,to learning the main characteristics of TCM syndrome differentiation of Sepsis,and finding its impact on the prognosis of patients, Then use of TCM theory to diagnose and treatment timely。MethodsCollect 86 qualified cases,combine with symptoms from book("traditional Chinese medicine diagnosis","traditional Chinese medicine clinical syndromes ") and clinical performance,then compile into a questionnaire,including patients basic information,such as sex,age,past medical history and so on;symptoms,tongue condition and pulse condition form four diagnostic methods of TCM。According to the diagnosis of syndromes standards to sum Upo Cases will be collected into the database,using statistical analysis software SPSSll.5 to analysis。Results1.According to the incidence of Sepsis patients:(1) Along with the growth of the age is,the number of sepsis has gradually increased。It shows the age is one of the most effective risk factor spredicting the incidence and death of sepsis and severe sepsis;(2) In all the infections which cause sepsis respiratory infections is the highest,it shows that respiratory infections (including upper respiratory tract infection and pulmonary infection) are closely related with the occurrence of sepsis;The positive rate of pathogen detection is not high,but gram-positive bacteria and fungi ratio is higher; (3) The proportion of the patients with coagulopathy is high,it indicate that clotting abnormalities in septic patients has high morbidity;(4) Patient's age and APACHEⅡscore have higher relevance,the higher the age and APACHEⅡscore are,the higher the mortality rate will be。2.From the characteristics of TCM Syndrome:fu-shi syndromes,the blood stasis syndromes,the yin-deficinecy syndromes are the common syndromes of sepsis。(1) The main symptoms of fu-shi syndromes are:fever,heat with wuhan,chills,dry mouth and pain mouth,cough,yellow sticky sputum cough,distension,abdominal pain,dry stool,red or red drop tongue,yellow greasy moss,slip a few clock,and so on;(2) The main symptoms of the blood Stasis syndromes are:fever,for dry mouth but do not want to swallow rinse water,cough, Physical pain,stabbing pain,night-time increase,Subcutaneous ecchymosis, bluish or ecchymosis tongue,Shibuya pulse,thready pulse or taut pulse, hematemesis,hematochezia,accompanied by abnormal blood coagulation:(3) the main symptoms of the yin-deficinecy syndromes are:low,cough,dry mouth, feverish sensation over the five centers,Pale chlorosis,red and dry tongue, less fur,rapid,thready pulse。3.The influence factor of prognosis:TCM syndrome and the APACHEⅡscore of the severity of disease are the influencing factors on the disease prognosis。ConclusionsFu-shi syndromes,the blood-stasis syndromes,the yin-deficinecy syndromes are the common clinical syndromes of sepsis,and every syndrome is not exist alone,but interlaced。TCM syndrome and the APACHEⅡscore of the severity of disease are the influencing factors on the disease prognosis。...
