Objective: To study the Tiotropium Bromide Inhalation therapy in stable chronic obstructive pulmonary disease (COPD) treatment.Methods: October 2008-January 2009 in our hospital ,50 cases of stable stageâ… ,â…¡level in patients with COPD, were randomly divided into Experimental group and control group. experimental group Tiotropium bromide inhalation powder dry (18ug, day 1), the control group of oral theophylline sustained-release tablets, each 0.1, 2 times per day, treatment for 12 weeks. In the treatment of 2 weekends, 6 on weekends, 10 weekends, a total of follow-up 4 times 12 weekends, before treatment, treatment for 6 weeks and 12 weeks .these patients were measured by lung function, blood gas analysis. Patients aged 35 to 75 years old, in line with the Chinese Medical Association, 2007 respiratory disease diagnostic criteria credits, exclusion of patients with bronchial asthma, bronchiectasis, congestive heart failure, tuberculosis, two weeks before the trial there is systemic infection, a serious heart, liver , kidney, blood system, nervous system diseases, glaucoma, Tiotropium Bromide on or atropine allergies, pregnant women and lactating women.Results: The completion of the treatment of COPD patients with 9-week experimental group25 cases, 25cases of the control group, no lost cases falling between the two groups by gender, age, course of disease, smoking history, height, weight, breathing, cardiac rhythm, pulmonary function parameters,no significant difference(Table1)(P>0.05). Two group effects:showed that 6weeks after drug treatment and 12weeks after the test group of patients with PO2, FEV1, FEV1/FVC(%) and FEV1(%) value of the mean compared with the control group were significantly different (Table 2).Conclusion: The experimental group, COPD patients with daily inhaled tiotropium bromide 18ug can significantly improve the patient's lung ventilation and ventilatory function, with the control group of patients with oral theophylline sustained-release tablets each of 0.1, compared to twice-daily efficacy significantly, and easy to use, no tolerance, only dry mouth as side effects. Theophylline for methyl xanthine derivatives, and its oral sustained-release preparations are commonly used in long-term treatment of COPD. In addition to the role of bronchodilator ,theophylline can also stimulate the respiratory center, lower pulmonary artery resistance, which combined have the COPD patients with pulmonary heart disease useful. However, theophylline's therapeutic window is very small, higher than the limit when the plasma concentration will appear obvious adverse reactions. Tiotropium Bromide bronchodilation effects are stronger than oral and less adverse reactions than theophylline.
