Diagnostic Values Of VEGF, MMP-2 And TIMP-2 In Differentiating Between Tuberculosis And Malignant Pleural Effusions

[Objective]The aim of this study was to explore the clinical validity of vascular endothelial growth factor(VEGF) and matrix metalloproteinase 2(MMP-2) and tissue inhibitor of metalloproteinase 2(TIMP-2) in discriminating between tuberculosis pleural effusion and malignant pleural effusion.[Methods]34 patients with tuberculous pleural effusion and 35 patients with malignant pleural effusion in the Affiliated Hospital of the Qingdao University Medical College,from March 2008 to July 2008,were the study subject.The VEGF,MMP-2 and TIMP-2 levels of pleural effusion were detected by enzyme-linked immunosorbent assay.The sensitivity and specificity of VEGF,MMP-2 and TIMP-2 were obtained through the receiver operator characteristic curve.All the data was delivered through means value±standard errand((?)±s).We analyzed the dita though T test,One-way ANOVA and Bivariate Correlations,it has significant difference when P＜0.05.[Results]1.In the group of malignant diseases,the average value of VEGF,MMP-2 and TIMP-2 were 527.3±204.8pg/ml,352.4±120.6ng/ml and 257.6±60.2 ng/ml,respectively.In the group of benign disease,the average value of VEGF,MMP-2 and TIMP-2 were 236.5±68.7pg/ml, 184.4±68.0ng/ml and 165.8±18.4ng/ml,respectively.The VEGF,MMP-2 and TIMP-2 of malignant group were significantly higher than that of tuberculous group(P＜0.01).2.The value ofVEGF,MMP-2.and TIMP-2 in malignant pleural effusion in different pattern of lung cancer had no difference.3.The MMP-2/TIMP-2 ratio had not difference in malignant group and tuberculous group(P＞0.05).4.In single determination,the sensitivity of VEGF,MMP-2 and TIMP-2 were 91.4%, 80%and 85.7%,respectively.The specificity were 88.6%,82.4%and85.3%,respectively;In combined determination,the sensitivity ofVEGF + TIMP-2 was 98.87%,and the specificity was 75,58%;The sensitivity of MMP-2 +VEGF was 98.2%and the specificity was 73.12%;The sensitivity of MMP-2+TIMP-2 was 97.14%,the specificity was 70.29%;The sensitivity of VEGF +MMP-2+TIMP-2 was 99.75,the specificity was 62.27%.5.VEGF level was positively correlated with the level of MMP-2 and TIMP-2 in two groups.[Conclusion]1.VEGF,MMP-2 and TIMP-2 could be used as valuable parameters for the differentiation of tuberculous pleural effusion from malignant pleural effusion.2.The combinated detection of VEGF,MMP-2 and TIMP-2 has importantly diagnostic values in differentiating between tuberculosis and malignant pleural effusion.3.The MMP-2/TIMP-2 ratio has little diagnostic values in differentiating between tuberculosis and malignant pleural effusion.
