Fix The Lens And Use Phacoemulsification From The Pars Plana To Treat The Dislocation Of Lens

Objective To evaluate the clinical effects of the treatment of the dislocation of lens by fix the lens and use phacoemulsification from the pars pianaMethods 17 patients(20 eyes) of the dislocation of lens were included in This study.For all patients,the rigidity grade of the nucleus of lens were over theâ…¡grade.The patiens were divided into two groups.the one was experimental group:To the 8 patients(10 eyes) of dislocation of lens,after the manipulation of PPV,inject the Liquid Perfluorocarbon to float upward the lens of dislocation to the approximate primary location,then use the appliance to fix the lens from the pars plana and carry into the practice of phacoemulsification.Pay attention to keep integrality of the lens capsule in the operation.Resect the lens capsule after resect the nucleus of lens completely.At last,implant the suture fixation of intraocular lens.The other was Control group:To the 9 patients(10 eyes) of dislocation of lens,after the manipulation of PPV,resect the lens of dislocation by the cutting head of the PPV instument.At last,implant the suture fixation of intraocular lens.We noted the course of the oparation of two groups respectively.The date coollected included the intraocular pressure(IOP),the events of the lens of dislocation that the fragments or the nucleus escape or fall into the vitreous chamber,and the duration of surgery of removing the lens in the operation.And it also included the condition of the corneal edema afer the surgery,the intraocular pressure(IOP) afer the surgery,and the visal acuity and the other complications after surgery.Results experimental group:To the 8 patients(10 eyes),we all used the ACM as the irrigator in the operation.1 eye was observed that the fragments or the nucleus of the lens escaped or failed into the vitreous chamber in the operation,the average duration of surgery of removing the lens was 2.67 minute.The visal acuity of 8 eyes was improved after the operation,and the visal acuity of 2 eyes was unconverted or declined.The intraocular pressure of 2 eyes was out of control and carried on the surgery of trabeculectomy.The grade 4 of the corneal edema was observed in 0 eye after the operation.And the detachment of the choroid was observed in 2 eyes.Control group:To the 9 patients(10 eyes),we all made the conventional three incisions of the pars plana.6 eye was observed that the fragments or the nucleus of the lens escaped or failed into the vitreous chamber in the operation,the average duration of surgery of removing the lens was 6.80 minute.The visal acuity of 7 eyes was improved after the operation,and the visal acuity of 3 eyes was unconverted or declined.The intraocular pressure of 3 eyes was out of control and carried on the surgery of trabeculectomy.The grade 4 of the corneal edema was observed in 1 eyes after the operation.And the detachment of the choroid was observed in 1 eye.We analysed the datas by the SPSS 13.0 and obtained the results:1.The contrast of the fragments or the nucleus of the lens escaped or failed into the vitreous chamber in the operation between two groups has statistic difference (P=0.029);2.The contrast of the average duration of surgery of removing the lens in the operations between two groups has statistic difference(P=0.000);3.The contrast of improvement of visal acuity afer the operation between two groups has no statistic difference(P=0.684);4.The contrast of the intraocular pressure was out of control and carried on the surgery of trabeculectomy afer the operation between two groups has no statistic difference(P=0.606);5.The contrast of the corneal edema afer the operation between two groups has statistic difference(P=0.013).Conclusions The treatment of the dislocation of lens by fix the lens and use phacoemulsification from the pars plana is a feasible modus operandi.It has fewer complications in the operation,shorter duration of surgery and few complications after the operation.
