Objective To investigate APACHE-Ⅱ,APACHE-Ⅲscoring systems in the early prediction of severe acute pancreatitis(SAP)and the severity of illness and prognosis value.Methods The scores of 132 cases of SAP evaluated by APACHE-Ⅱand APACHE-Ⅲscoring systems were retrospectively analyzed.The Pearson correlation and Spearman rank correlation test were computed to assess the following prognostic indicators:local complications,infections,organ failure,sur- gical drainage,death,the number of days of fasting and hospitalization days.Results APACHE-Ⅱand APACHE-Ⅲscores were respectively range from 0~27,10~104;The average scores were 9.80±6.44,41.37±22.72 respectively.APACHE-Ⅲscores in organ failure group were significantly higher than the group without organ failure(P <0.05),The incidence of organ failure in higher score group was higher than the lower score group(P< 0.05);The two scoring systems were no significant difference in gender. APACHE-Ⅱwas correlation with the number of days of fasting, organ failure and death. APACHE-Ⅲwas correlation with hospitalization days, organ failure, death.Conclusions APACHE-Ⅱand APACHE-Ⅲscoring systems could be used as an important reference in evaluating the severity and prognosis of SAP.The higher of the score,the more serious of the severity,and the higher of the incidence rate of complications,the worse of the prognosis.APACHE-Ⅱhas advantage in forecasting organ failure and death comparing to APACHE-Ⅲ.
