Objective To investigate the applied value of APACHE II, Ranson and Balthazar scoring system for predicting the severity and prognosis, and guiding clinical therapy in severe acute pancreatitis (SAP)Methods A total of 94 patients with SAP who were admitted into the number two hospital of Shanxi medical university from September 1999 to September 2009 were analyzed with A PACHEE, Ranson and CTIS criteria retrospectively.Results To APACHEâ…¡scores the incidence of organ failure in higher score group was higher than that in lower score group (P< 0.01).Patients treated with operation and conservatively showed no difference in Ranson score (P> 0.05).The CTIS scores of Balthazar scoring system were closely related to the clinical therapy and prognosis. The incidence of organ failure and chances of operation in the higher score group were significantly higher and more than those in the lower score group (P< 0.01).Conclusion APACHEâ…¡and Balthazar scoring system could be used as an important reference in evaluating the diagnosis,treatment and prognosis of SAP, especially be applied together. but the value of Ranson criteria is no t stable to indicate surgical agents independently.
