Objective: Breast cancer is the one of the most common cancer in women. It is a serious interfere to women's life and health. Although tradition therapeutic tool can make the tumour tissue extinct or relieve, most of the tumour will be recrudescence or metastasis in a few monthes or year. Many researches suggest that cancer stem cells may be the priming cells to retain the cancer cells to grew viciously and they are ideal spiritual state of the recrudescence or metastasis. The content of cancer stem cells has the relationship with malignant degree and prognosis. The recent investigations have discovered that solid tumours which include the breast cancer have the cancer stem cells. The high express CD55 can be the marker of breast cancer stem cells. The CD55 is the decay accelerating factor(DAF).It combines with C3 convertase to prevent C3b to deposite and to repress the attack coat compound to form. The CD55 can make the tumour cells to escape from the complement attacking. The researchs have discovered that CD55 is over expressing in tumour basilaris substantia and normal connective.It means that the CD55 takes part in the change of tumour microenvironment.So the CD55 is associated with the occurrence, invasion, and metastasis of tumor. This study want to investigate the expression of CD55 and its correlation to clinical pathology factors such as the size of the tumor, clinical stage, the number of metastasis in axillary nodes, tumor histology grade, ER, PR, C-erbB-2, micro-metastasis in bone marrow and prognosis of the patients, and the relationship between CD55 expression and development, relapse, metastasis and meaning of estimation of prognosis of breast cancer, and to direct individualized treatment.Methods: To choose 49 women patients with primary breast cancer, who got hospitalization in the 1st surgery department of the 4th hospital of Hebei Medical University from the Dec. 2002 to Dec. 2003. The select cases are the first time attendance, and lack of any treatment. To choose another 10 specimens of para -tumor breast tissues as contrast. The follow-up period was from twenty eight months to sixty months .The median follow-up period was 56 months.The samples were detected in the level of albumen by SP immunohistochemistry. The clinical data of patients with pathological type, histological grade, axillary lymph node metastasis and C-erbB-2, and other factors were combined to do correlation analysis. the pathological tumor types were according to surgical biopsy specimen and pathology report in our hospital. The clinical data of patients, lymph node metastasis and C-erbB-2, and other information came from medical records. The consequence of immunohistochemistry was assessmented by observatiting 2000 cells at least in 20 high power eyeshots.SPSS13.0 software andχ2-test were used to analyze the relationship between CD55 expression and clinicopathologic characteristics. The method of log-rank was used to analyze the differences of futher life span between the three groups. Theα= 0.05 level is the size of test.Results:1. Most of the specimens of breast cancer was positive staining by CD55 antibody. The positive rate of CD55 in the epithelial cells of breast cancer tissues was significantly higher than that in para -tumor breast tissues ( 65.3% vs 0%) .2. The expression of CD55 in the breast cancer tissues was increased with the augmentation of clinical stage(P=0.008). It meaned that higher clinical stage was with the higher expression of CD55.3. The expression of CD55 in the breast cancer tissues was increased with the augmentation of tumer size(P=0.039).4. The expression of CD55 in the breast cancer tissues was increased with the multiplication of lymph nodes of metastasis(P=0.001)and the expression of CD55 in the breast cancer tissue with the lymph nodes of metastasis was higher than the patients without the lymph node sof metastasis.5. The expression of CD55 in breast cancer tissues was increased with the augmentation of C-erbB-2. 6. The expression of CD55 in the breast cancer tissues had a close-up relationship with micro-metastasis in bone marrow(P=0.002). The probability of positive staining of CD55 was higher than negative staining in micro-metastasis in bone marrow. There was the correlation between the patients with positive expression of CD55 and the micro-metastasis in bone marrow or poor prognosis.7. There was no relationship between the expression of CD55 in the breast cancer tissues and age of the patients(P =0.309), ER(P =0.824), PR(P =0.666)and the histology grade(P =0.750).8. The expression of CD55 in the breast cancer tissue had positive relationship with long-term survival rate(P=0.013). Their were conspicuous differences between negative staining group, midrange positive staining group and strong positive staining group. The survival rate of strong positive staining group was the lowest. It had the significant difference(P=0.005).Conclusions:1. Most of the specimens of breast cancer tissues are highly expressing the CD55 and normal breast tissues are lowly expressing the CD55.2. The expression of CD55 in the breast cancer tissues have positive relationship with the size of the tumor. The expression of CD55 is increased with the augmentation of tumer size.3. The expression of CD55 in breast cancer tissue have positive relationship with the clinical stage. The expression of CD55 is increased with the augmentation of clinical stage.4. The expression of CD55 in breast cancer tissue have positive relationship with the axillary nodes metastasis. The expression of CD55 is increased with the multiplication of lymph nodes of metastasis and the expression of CD55 of the patients with the lymph nodes of metastasis is higher than the patients without the lymph nodes of metastasis. It means that the patients with the positive expression of CD55 have a higher probability in the lymph nodes of metastasis.5. The expression of CD55 in the breast cancer tissue have positive relationship with the expression of C-erbB-2. The expression of CD55 is increased with the augmentation of the expression of C-erbB-2.6. The patients with positive expression of CD55 have more chances with distant metastasis and poor prognosis.7. The long-dated survival rate of strong positive staining group is notable lower than negative staining group.8. CD55 may be one of the factors of prognostic, relapse, metastasis and malignant degree of breast cancer.
