| Objective:To understand the status and characteristic of late vitamin K deficiency bleeding in China.Method:By means of the Chinese computer retrieve system and vitamin K deficiency as Key word or subject heading ,look for the total documents of late vitamin K deficiency bleeding from 1989 till 2008(twenty years).The database include National Knowledge Infrastructure (CNKI),VIP,wanfang,Chinese Biomedica(lCBM).Then make Excel table with Relevant data to Analysis.Results:The total of 21894 cases of late vitamin K deficiency bleeding were obtained from 656 pieces of eligible documents. Patients characteristics: The boys were dominated(68.9%), infants from rural area were more(84%), The exclusive breast-fed infants were 19416 cases(90.1%) , Incidence areas were broad:guangdong,shandong,henan,zhejiang,jiangsu were higher,neimenggu,gansu,ningxia were lower, The peak age was 4~8 weeks (12654 ,73 %) ,1040(6 %) cases were inferior to 4 weeks and 3640 (21 %) cases went beyond 8 weeks. During the risk factors, un-prophylactic use of vitamin K infants were 93.9%,Prior to the onset of LVKDB ,infants who had respiratory tract infection were 25.2%, infants who had diarrhea were 25.6%, who accompanied by liver or gallbladder diseases were 13.9%, who received antibiotic treatment were 34.4%;Clinical manifestations include: Gastrointestinal bleeding(16.7%),Mucocutaneous hemorrhage ( 23.1% ),Injection site hemorrhage(43.4%)and intracranial hemorrhage et al,the later happened more(90.4%); Prognostic Analysis: The majority were recovered or turned bette(r79.7%),the least were given up(8.16%) or died(12.1%); Follow-up analysis:the follow-up rate was 20%, physical or mental retardation were more often(13%),Secondary epilepsy were relatively low(9%).Conclusions: In China, the incidence of late vitamin K deficiency bleeding is grim ,Breast-feeding infants happen more; un-prophylactic use of vitamin K,antibiotic treatment,infections(diarrhea,respiratory tract infection et al),liver or gallbladder diseases and so on are all risk factors;The rate of occurring intracranial haemorrhage is high; Preventive measures should be taken. |
