ObjectiveFungal rhinosinusitis includes noninvasive fungal sinusitis (mycetoma or fungus ball, allergic fungal sinusitis) and invasive fungal sinusitis (acute, chronic, granulomatous). The distinction between the two categories is whether the fungal invasion has been found in the sinus mucosa, submucosal, vascular, bone tissue, and so on. Without early treatment, the invasive fungal sinusitis can progress rapidly, poor prognosis, morbidity and mortality are very high. To our knowledge, few research about experimental fungal sinusitis study in rabbits. There is no report about invasive fungal sinusitis in experimental model. The aim of our study is to confirm the methods to establish the experimental model of fungal sinusitis in rabbits, to research the methods to establish the experimental model of invasive fungal sinusitis in rabbits and the curative effect of ostia enlargement operation. The present model of experimental models seem to be reproducible and suitable for further studies of the development of fungal sinusitis.MethodsThe methods to establish the experimental model of non invasive fungal sinusitis in rabbits.A fumigatus was inoculated into the New Zealand white rabbits maxillary sinus. the treatment of the natural ostium and mucosa was different in different group. the control groups without fumigatus. At days 14 and 28 after inoculation, both maxillary cavities were submitted to endoscopic examination of the mucous membrane and secretions. Samples for bacteriologic and mycological examination were taken from each maxillary sinus. All rabbits were painlessly sacrificed 28 days after inoculation. Mucosa and bone specimens of the maxillary cavities were taken for pathologic examination. The methods to establish the experimental model of invasive fungal sinusitis in rabbits.A fumigatus was inoculated into the New Zealand white rabbits maxillary sinus. the treatment of the immunosuppressant was different in different group. the control groups without immunosuppressant. All rabbits were divided according to the presence of open ostia operation and ostia enlargement operation. 6 weeks after operation , both maxillary cavities are submitted to endoscopic examination of the mucous mermbrane and secretions. Samples for bacteriologic and mycological examination were taken from each maxillary sinus. Mucosa and bone specimens of the maxillary cavities were taken for pathologic examination. Observed the curative effect of two groups after 6 weeks.ResultsThe methods to establish the experimental model of non invasive fungal sinusitis in rabbits. At days 14 after inoculation , The positive rate of fungal culture of the group(A fumigatus and ostial obstruction) was 88.9%(8/9 sides), At days 28 after inoculation, The positive rate of fungal culture of the group(A fumigatus and ostial obstruction) was 77.8%(7/9 sides). all histologic examination of the maxiltary sinus mucosa without fungal invasion. The methods to establish the experimental model of invasive fungal sinusitis in rabbits. At days 28 after inoculation, The positive rate of fungal culture of the group with CTX (A fumigatus and ostial obstruction) was 100.0%(8/8sides); The positive rate of fungal culture of the group with DEX(A fumigatus and ostial obstruction) was 100.0%(8/8sides); The positive rate of fungal culture of the group without Immunosuppressant (A fumigatus and ostial obstruction) was 87.5%(7/8sides). all histologic and pathologic examination of the maxillary. all histologic and pathologic examination of the maxillary sinus mucosa without fungal invasion. The curative effect of ostia enlargement operation of the experimental model of non invasive fungal sinusitis in rabbits. After established the experimental model of non invasive fungal sinusitis in rabbits. all rabbits were divided according to the presence of open ostia operation and ostia enlargement operation. 6 weeks after the operation. both maxillary cavities were submitted to endoscopic examination of the mucous mermbrane and secretions. Samples for bacteriologic and mycological examination were taken from each maxillary sinus. Mucosa and bone specimens of the maxillary cavities were taken for pathologic examination. The effective rates of ostia-enlarged operation was 83.3%(10/12sides) ,and for ostia—opened was 33.3%(4/12sides).ConclusionExperimental non invasive maxillary fungal sinusitis can be successfully created by fumigatus inoculation with an blocked ostium.Experimental invasive maxillary fungal sinusitis not be created by fumigatus inoculation with an blocked ostium and with Immunosuppressant, the methods to establish the experimental model of invasive fungal sinusitis need to be explored further. The curative effect of fungal sinusitis is better with ostia—enlarged operation than open ostia operation...
