Background:the incidence rate of burkitt lymphoma in china is quite rare.Burkitt lymphoma predominantly involving the stomach is even rare.Since the clinical symptoms and signs resemble those of gastric cancer,primary diagnosis of gastric lymphoma is generally difficult.The prognosis of patients with advanced stage of malignant lymphoma is poor;however,chemotherapy may be more effective than it is for gastric cancer.Burkitt lymphoma/Burkitt cell leukemia is a B-cell neoplasm which often presents with extranodal disease and occurs most often in children(endemic, sporadic) and immunocompromised hosts.Endemic cases,usually from Africa,involve the facial bones or jaw of children,mostly containing EBV genomes.Burkitt lymphoma may also involve other structures,including the liver,adrenal glands, stomach,intestine,pancreas,salivary glands,thyroid,testicles,heart.etc.Histologically, it is a poorly differentiated lymphocytic lymphoma.Macrophages with abundant clear cytoplasms are usually scattered uniformly throughout the tumor resulting in the"starry sky"effect.A brief introduction of the Case::A 32-year-old man complained about having upper abdominal pain for four months.The patient presented with continual mild upper abdominal pain 4 months ago,and occurred mostly after a meal.The pain had been getting worse over the past week,and could not be relieved by eating or drinking hot liquids.Physical examination:R:20bpm,P:86bpm,BP:110/82mmHg,T:37.1℃, Weigt:73kg,Heiht:175cm,other physical examination findings were normal. Gastric biopsy IHC LCA+++,CK(pan)+++,CD20+++,CD3-,CD79a+++,Ki67+++>90%, CD5-.Gastric biopsy and IHC suggests burkitt lymphoma of stomach.There is a single extranodal tumour and there are no nodal lymphoma in any area of the body,so the diagnosis is burkitt lymphoma Stage 1.The patient tolerated well and got a good recent effect after five cycles of R-CHOP therapies.Lesions observed by gastroscopy became smaller from 4×5cm to only remain a linear scar。And meanwhile after the R-CHOP therapy the pathology of gastric biopsy find no more tumor cells.
