Preliminary Study On Pancreatic Carcinoma Invasion To Peripancreatic Vessels: Evaluation With Multislice CT Angiography

ObjectiveThrough the multislice CT angiography examination to explore the relationship between the pancreatic tumor site,size and invasion peripancreatic vessels,image changes from invasion vessels,and to discuss the accuracy of MSCTA in evaluating the invasion of pancreatic carcinoma to surrounding vessels,and discuss the significance of CTA.MethodsPatients with pancreatic carcinoma were scanned by Siemens Sensation 64 detector row computed tomography(CT).The primitive axis images obtained in arterial phase and portal phase were transferred to post-processing workstation to make CT angiography of the major peripancreatic vessel.The state of the peripancreatic vessel encroached by pancreatic carcinoma were distinguished and evaluated in three-dimension.Correlation was made between MSCTA findings and surgical results.ResultsAmong the total,176 vessels of 22 patients underwent operation were involved,41 vessels were invasive and 135 vessels were non-invasive affirmed with CTA.40vessels were invasive and 136 vessels were non-invasive were affirmed in the operation.5 vessels were misdiagnosed and 4 vessels were undiagnosed.For display of peripancreatic vessel invaded with CTA,the sensitivity was 90.0%,and the specificity was 96.3%,positive predictive value was 87.8%,negative predictive value was 97.0% The overall accuracy of MSCTA for vessels invasion was 94.9%(Chi2square x~2=128.9, P=1,the assessment of peripancreatic vessel invaded by pancreatic carcinoma between with preoperative CTA and surgery had no statistical difference;Kappa value of 0.856(K> 0),that is,the results of the assessment MSCTA with better surgical outcome of consistency).The pancreatic tumor site,size and invasion peripancreatic vessels have relationship.The images are different between peripancreatic artery and peripancreatic veiri。ConclusionThe prediction of peripancreatic vessel invaded with MSCTA was accuracy and had prospective guide significance for surgery.
