Objectives : To evaluate the clinical value of 320 row spiral CT angiography in the preoperative assessment of vascular involvement and resectability of pancreatic carcinoma.Methods:Ninety-eight cases of pancreatic carcinoma confirmed by surgical pathology were retrospectively analyzed.They were divided into three groups: resectable,borderline resectable and unresectable.The results were compared with those of operative findings.Results:37 cases were diagnosed resectable pancreatic carcinoma of ninety-eight patients,which were regarded as resectable group,38 cases in borderline resectable group and 23 cases in unresectable group.Compared with surgery,spiral CT carries an accuracy of 92.2%,sensitivity of 89.9% and specificity of 92.8% in diagnosis of artery infiltration,and an accuracy of 96.9%,sensitivity of 95.6% and specificity of 97.3% in diagnosis of vein infiltration.The diagnosis accuracy of CT in the three groups were 97.3%?86.8%?91.3%.Conclusion:CT has great value for diagnosis of pancreatic carcinoma and for assessment of major blood vessel involvement.CT can help to assess the resectibility of tumor before operation.
