| Objective:The study was performed to dynamicly view aeteria coronaria endotheliocyte, cadiocyte's defect and myocardial functional impairment of early sepsis rats,and to briefly discuss the correlation and possible mechanism.The sepsis model was induced by cecal ligation and puncture.Method: 72 SD rats , female and male equally ,were randomly divided into three groups as follows: (1) normal control group (n=8); (2) sham operation group (n=32); (3) sepsis group (n=32).Sham operation and sepsis group were further divided respectively into 3h, 6h, 12h and 24h subgroups after CLP,each subgroup of sham operation and sepsis group had eight rats.All rats were measured left ventricular systolic pressure(LVSP), LV ventricular pressure maximal rate of fall(-dp/dtmax),LV ventricular pressure maximal rate of rise(+dp/dtmax) through left common carotid artery after anesth; detected the changes of E-selectin , endothelin-1, NO and treponin I in blood serum, endothelin-1 and NO in myocard homogenate,the number of circulating endothelial cells; observed damage of aeteria coronaria endotheliocyte and cadiocyte using electron microscope.Results:1. LVSP,+dp/dt max and -dp/dt max of sepsis group obviously decreased at 3 hours after CLP,at 6 hours reached minimum(71.0±6.5mmHg , 3702.1±392.6 mmHg/s,-1169.7±159.3 mmHg/s), significantly less than normal and sham operation group's(p<0.01).After that, them begained to rise,but still less than that of normal and sham operation group(p<0.05).2. The serum levels of cTnI in sepsis group obviously rised at 3 hours after CLP,reached to peak value(1.489ng/ml) at 12 hours point,after that started to drop down,but still exceed normal and sham operation group's (p<0.01).3.The level of NO and ET-1 in serum and myocard homogenate obviously rised,at 12 hours after CLP reached maxlmum, significantly larger than that of normal and sham operation group(p<0.01),and kept high level. The proportionality of NO and ET-1 gradually and persistently decreased after CLP, significantly lower than sham operation group(p<0.01).4.At 3 hours after CLP,the serum level of E-selectin reached 0.616±0.153 ng/ml, exceeded normal and sham operation group's(p<0.05),at 12 hours point got to peak(6.248±0.679 ng/ml).5. The number of CEC in spsis group was more than that of normal and sham operation group at 3 hours after CLP, reached maxlmum(10.938±1.393/ 0.9) at 12 hours point,and then begained to decrease,but at 12 hours after CLP was still larger than normal and sham operation group's(p<0.01).6. The damage of aeteria coronaria endotheliocyte and cadiocyte were gradually aggravated in sepsis through electron microscope,and the score of vascular endothelial cell gradually rised with time-lapse ,according to score standard of Li-ao burns.7. CEC and serum cTnI had obviously direct correlation, Kendall and Spearman coefficient correlation were 0.307 and 0.445, and regression equation were y( cTnI) =2.887+4.306x( CEC) . Conclusions: 1.The damage of endotheliocyte and cadiocyte were gradually aggravated with the development of sepsis, endotheliocyte's impairment may play an important role in sepsis.2. Myocardial functional disturbance and degree of injury of cadiocyte were not at equal,whose mechanisms may be rejection capability substance of myocard softening depressant effect in short term , cordis pumping blood capability were transiently elevated by compensation of neuroendocrine's feedback regulation, chondriosome's hyperplasy induced by ischemic and hypoxia in sepsis.3. The damages of VEC and cadiocyte had obviously direct correlation,which hints endothelial cell may play an important role in cardiac muscle cellular damage. |
