| Objective:To evaluate the effects of Chuankezhi Zhusheye on syndrome of Traditional Chinese Medicine(TCM),breath dyspnea index(BDI),lung function,exercise tolerance and airway inflammation in patients with chronic obstructive pulmonary disease(COPD) in moderate to very severe stages(StagesⅡ-Ⅳ).To assess the application value of induced sputum in patients with COPD in stable phase.Methods:60 patients with COPD(StagesⅡ-Ⅳ) in stable condition,according to the yang deficiency of spleen and kidney in TCM were enrolled.The patients were randomly divided into two groups(therapeutic group 30 cases and control group30 cases,respectively). The therapeutic group was treated by Chuankezhi Zhusheye combined with GOLD guideline treatment while the control group was treated with GOLD guideline only.Spirometric testing including forced expiratory volume in one second(FEV1),percentage of the predicted volume (FEV1%pred),forced vital capacity(FVC),6-minute walk distance,total cell counts and differential neutrophils(neutrophils%) in induced sputum were measured before and after treatment.Total syndrome of TCM score and BDI were recorded simultaneously.Results:1.The total effective rate of TCM syndrome score in the therapeutic group was higher than that in the control group(89.29%vs 62.07%,P<0.05) after 3 months treatment.2.The indices of lung function on FEV1 and FEV1%pred in two groups did not increased after 3 months treatment compared with baseline(P>0.05,respectively).But FVC in two groups increased significantly compared with baseline(P<0.05,respectively).The improvement in FVC was greater in the therapeutic group than that in the control group (P<0.05).3.Mean 6-min walk distance significantly improved after 3 months treatment compared with baseline,from 362.96±15.36m to 426.60±13.43m(P<0.05) in the therapeutic group and from 362.03±13.01m to 394.31±11.54m in the control group(P<0.05).The improvement in 6-min walk distance was greater in the therapeutic group than that in the control group (P<0.05).4.BDI in both groups significantly decreased 3 months later compared with baseline (P<0.05,respectively).It had greater decreased in the therapeutic group than that in the control group(P<0.05). 5.There were significant differences between baseline and 3 months later in total cell counts and neutrophils%in induced sputum in both groups(P<0.05,respectively).They decreased significantly in the therapeutic group than those in the control group(P<0.01, respectively).Conclusion:1.The results indicated that Chuankezhi Zhusheye combined with GOLD guideline treatment in the therapeutic group had the better effects than single treatment with GOLD guideline in the control group in the aspects of reducing syndrome of TCM score,relieving dyspnea,raising patients' exercise endurance and decreasing airway inflammation.2.Induced sputum technique as a non-invasive detecting method,can be used as an effective clinical and scientific researching approach on the assessment of airway inflammation,and can be used as an outcome measure of drug effectiveness. |
