【Objective】To evaluate the clinical results of laparoscopic transcystic common bile duct exploration for extrahepatic cholelithiasis.【Methods】We retrospectively analyzed the clinical results of laparoscopic common bile duct exploration for common bile duct stone performed on 111 cases between October 2006 and October 2008 of General sur of qilu hospital,Of which,60cases laparoscopic transcystic common bile duct exploration,while 51 cases had T-tube drainage of common bile duct.Clinical results of 111 patients who were followed up were reported.This group included 45 male and 66 female and age range was 35 to 65 years(mean 45y).Mean course of disease was 36.5 months.The operation time of the two groups was 161.8±72.7min and 168.0±78.1min.Amount of fluid of the two group was 4861.0±578.4ml and 9600±940.5ml.Postoperative hospital stay was4.4±1.8 days and 8.0±2.4days.Abdominal drainage time was 3.6±0.8 days 3.4±0.8 days.Long-time clinic results were analyzed according to letter,telephone and results of B-ultrasound CT.【Results】There was no significant difference in terms of operation time and abdominal drainage time between the two groups(P>0.05).Postoperative hospital Stay was shorter and fluid replacement was less in first group than that in drainage group.Postoperative bile leakage was seen in 4 cases(4/60)in closure group,and 2 cases(2/51)in drainage group(P>0.05).Retained bile duct stones were seen in 2 cases(2/60)in closure group,and 3 cases(3/51) in drainage group(P>0.05).Bile duct recurrent stones was seen in 4 cases(4/60),while 4 cases(4/51) in drainage group(P>0.05).Bile peritonitis was seen in 1 cases after T tube removal.There was no cases of postoperative extrahepatic duct stenosis in the two groups.【Conclusion】laparoscoplc transcystic common bile duct exploration for common bile duct stones is safe and effective...
