Objective: To compare the curative effect between the Laparoscopic transcystic common bile duct exploration(LTCBDE)with traditional common bile duct exploration(LCBDE)which to merge the common bile duct stones with the gallbladder stones.Discuss the feasibility and the experience from LTCBDE which to merge the common bile duct stones with the gallbladder stones.Methods: Retrospective analysis the clinical data of the 65 cases with the common bile duct stones with the gallbladder stones under the laparoscopy that come from our hospital.According to the operative methods,we make 2 groups,group1:LTCBDE(n=29),group 2:LCBDE(n=36),to contrast the curative effect of situation during operation,postoperative complications.Results: LTCBDE group included 29 cases,(there is 28 the successful operations),and the success rate is 96.6%.The reason of the failure case is that large amount of CBD stones and severe inflammation caused that it is difficult to take the stones out through the cystic duct,and perform the laparoscopic laparotomy.LCBDE group included 36 cases(there is 36 the successful operations).The postoperative complication include only one bile leakage and one residual stones.To compare with the LCBDE group,the LTCBDE group had a shorter operative time[126±30.386 vs 148.17±40.857(min)],less blood loss[20.71±16.762 vs34.17±16.279(ml)] and a shorter postoperative hospital stay[5.57±1.37 vs7.50±2.171(d)],(P < 0.05).There was no significant difference between the two groups on Hospital expenses[34287±8094 vs 33521±9553(yuan)] and postoperative complications(P>0.05).Conclusion: In conclusion,the LTCBDE can avoid postoperative T-tube drainage,shorten operative time,shorten hospital stay,and increase the life quality.The safety,feasibility,and short-term clinical benefits of LTCBDE deserves affirmation and praise.And we believe that such an approach deserves the promotion and extension because of its good clinical efficacy.
