The Analysis Of The Causes And The Majoy Influencing Factors Of 171 Perinatal Death

Objective:To find the main causes of perinatal death and learn the factors related to the death of hospitalized perinatal infant from 2006.10～2008.9in qilu Hospital,supply clue and basis for the prevention and control of the death and to evaluate the effect of perinatal care in the research of the intervention to the death of fetus.Give the suggestion for further prevention and decreasing the perinatal mortality.Methods:According to the MCH surveillance protocal of ShanDong province,collect the sum of hospitalized perinatal infant from 2006.10～2008.9in qilu Hospital,and the sum of death of hospitalized perinatal infant.The SPSS11.0 is used to analyze of the researth.The relations between categorical variables were examined by Chi-squared test.Results:1.There are 5476 hospitalized patients occupying perinatal stage in Shandong University Qilu Hospital during the period of 2006.10 to 2008.9,delivering 5541 perinatal infants.According the materia,171 perinatal death were reported.The average perinatal mortality is 30.86‰,in which 128 cases of stillbirth and 1 infant died during the labour,42were the neonatal death within the first seven days.The mortality rate accounted for 23.10‰,0.18‰,7.58‰,respectively,constitute more than 74.85%,0.58%,25.73%.2.According to the monitoring scheme of Shandong province,six factors were recognized as major reasons,which were maternal factor,placenta factor,umbilical cord factor,neonatal diseases,birth defects.Then further detail death cause was analyzed.From our data four main single causes were birth defects,maternal factor, premature,placenta factor and umbilical cord factor.3.Factors related with perinatal death:①The mortality of perinatal infant borned in the first quarter of a year was 34.31‰,the second,third and fourth quarter average birth perinatal mortality rate was 29.73‰.The mortality of perinatal infant borned in the first quarter of a year was higher than that borned in the other three quarter,but the difference was not significance(x~2=0.72,p＞0.05).②The mortality of perinatal infant which the lying-in women aged≥35 is 60.78‰,while the 30～34years old group is 32.56‰,the 25～29years old group is 20.07‰,the＜25 years old group is 56.34‰.Using x~2 test to compare 4 groups,and p＜0.05 was significantful.After that compare every 2 groups,and p＜0.05 was significantful.The result show that the 25～29 years old is the best age for child-borning.③The local mortality of perinatal infant is 19.94‰while the floating population mortality of perinatal infant is 103.30‰.The statiscal analyzation shows p＜0.05 with x~2 test.The city mortality of perinatal infant is 18.65‰while the country population mortality of perinatal infant is 83.98‰.The statiscal analyzation shows p＜0.05 with x~2 test.④The mortality of male infant is 31.51‰, female infant is29.77‰,there is no significant difference between the both sexes.⑤The mortality of premature perinatal infant is 68.72‰.The mortality of term birth perinatal infant is 0.41‰,there is significant difference between the two groups.⑥The mortality of perinatal infant have significance between low birth weight infants and the infants which birth weight＞2500g(x~2 =214.15,p＜0.05 ).⑦The mortality of singleton perinatal infant is 27.49‰and multifetation is 124.35‰.there is significance between the two groups.Conlusion:At present the ratio of perinatal infant death in Qilu Hospital is in high level. Birth defects is the most reason.There are many factors influence the kinds of pefinatal infant death.They are age,censue register,the pregnancy weeks,low birth weight infant and multifetation.
