Objective: To evaluate the clinical effect and safty in the treatment of the superficial bladder tumors and cystitis glandularis with holmium laser.To discuss the skills in the operative procedure and the prenvention of complications.Method: From October 2005 to September 2007,a total of 54 cases of transitional cell bladder carcinoma,including 42 male and 12 female,with clinical stage T1 to T2a,and grade I toⅡ,were treated with holmium laser.46 cases were primary and another 8 were recurrent. 47 cases were under epidural anesthesia and 7 cases were under local anesthesia . During surgery , all 54 cases retained the lithotomy position .The holmium laser's power was 10—30W. the small tumor(diameter<1.0cm) were vaporized directly, the large tumor (diameter>1.0cm) were fulgurated at the basilar part. During same period,and 4 cases of cystitis glandularis were resected by holmium laser transurethrally, including 1 male and 3 female.Results: All 54 cases were operated successfully without severity complications like obturator nerve reflection,bladder perforation and overhydration. No one case required blood transfusion. The mean operation time was 30min (range:10~70min), the mean catheterization time was 3d (range:1~5d),the mean time in the hospital after the operatin was 4d (rang:3~ 7d) . and 4 cases tumors recurrence were found during 3 to 12 months follow up period. And all 4 cases of cystitis glandularis were resected successfully.Conclusion: Holmium laser resection of the superficial bladder tumors and cystitis glandularis is a safe,effective, simple method with less side effects and low recurrence rate.
