Objective: To observe the efficacy of Amlodipine, Terazosin and its combination effect on the IPSS scores in the patients with both hypertension and lower urinary tract symptoms, and to define the different prescription for the patient with both hypertension and lower urinary tract symptoms.Methods: A population-based epidemiological investigation was conducted according to the criteria below:the subjects above 50-year old,male people with both hypertension and lower urinary tract symptoms with IPSS socres above 10 points in anqing rural areas in anhui province,the total subject number is 360,collected the datas about IPSS, maximal urinary flow rate(Qmax),average urinary flow rate(Qave),sbp,dbp and other demographic charactristics were determined at baseline .all the sujects had been divided into 3 parallel groups in a randomized clinical trial,which were Amlodipine/5 mg, Terazosin/ 2mg and the combination group respectively. And the latter two groups had been give the initial dose of Terazosin 1mg per day in the fisrt 4 days.T-test had been applied for the efficacy of IPSS scores reduction, ANOVA and mutiple linear regression had been used to the efficacy analysis between groups.P<0.05 had been defined as having statistical significance,while P<0.01 had been defined as having notable statistical significance.Results: After 4 weeks treatment,all 7 sub IPSS scores had significant reduction.and the combination group were much better than Terazosin (P=0.034) Amlodipine(P=0.035) on reducing the frequent micturition,and as well as better than Amlodipine in straining urination(P=0.024)。The combination were better than single therapy alone in reducing the nocturia frequency,but didn't got a statistic significance. In the total IPSS score reduction, the efficacy of Amlodipine was no less than the combination and Terazosin groups in mild to moderat patients.while in the severe patients , the combination therapy showed a advantage over Amlodipine (95%CI: 0.68-6.48) and Terazosin (95%CI:-0.18-5.62) .In relieving the symptoms of overactive bladder (OAB), Amlodipine's efficacy was no less than combination and Terazosin in the mild to moderate patients with OAB. Terazosin had a better efficacy in relieving the symptoms of bladder outlet obstruction(BOO) than Amlodipine and combination group in mild to moderate BOO patients, where the combination group showed a privilege over Amlodipine (95%CI:1.23-5.39) and Terazosin (95%CI: 0.18-4.63) in the severe patients.Conclusion: all three groups had significant improvement in the seven IPSS scores, Amlodipine was no less than Terazosin and combination groups on relieving symptoms in the hypertension patients with mile to moderate OAB,The combination therapy had a better efficacy on relieving the severe lower urinary tract sypmtoms accompanied BOO than Amlodipine and Terazosin.
