Background:The sphincter of Oddi dysfunction(SOD) is present on a common clinical disease,tend to divide it into sphincter of Oddi dyskinesia and sphincter of Oddi stenosis.The former is SO functional disorder while the latter is SO structural abnormality.SO dyskinesia may develop as SO stenosis.therefore, Accurate and early identification of SO dyskinesia is particularly important in the diagnosis and treatment of SOD. But clinically it is difficult to distinguish between the two. Sphincter of oddi manometry (SOM) is the most direct and objective way to evaluate SOD, and the result is the gold standard for diagnosis of SOD.SOM is helpful in clinical prac- tice to distinguish between the sphincter of Oddi dyskinesia or stenosis, is the best way to to study the function of SO.The main signs SOD is currently recognized as: abnormally elevated sphincter basal pressure, delayed contrast agent emptying, sphincter Spasm, which leads to biliary dilatation. However, due to species differences and complexity of neurohumoral regulation, the pathogenesis and neurohor- monal regulation of SOD is not clear.Also, lack of direct and quantitative methods in diagnostic, no specific and effective means in treatment, makeing SOD clinically a common and difflcult disease. Therefore, building a mature and stable animal model of SOD and to observe the different effects of drugs on SO become very important, which are bound to provide a reasonable theoretical basis for related research like pathogenesis, diagnosis and treatment of SOD.Domestic and foreign literatures on clinical study of the SOD are nuemerous.Immunohistochemistry confirmed that SOD relates to reducement of inhibitory neurotransmitter nitric oxide synthase (NOS) It has been clinically proven that morphine can cause movement disorders and spasm of the sphincter of Oddi. On clinical, mor- phineneostigmine provocation test is often used to select in patients with suspected SOD.but relevant animal studies of SOD is still relatively small.Animal Models of SOD is still in the exploratory stage. Da cheng qi tang are common herbs treating biliary, pancreatic diseases, and made good curative effects.Magnesium sulfate is clinically recognized as smooth mus- cle relaxation drug. Based on the above understanding, this study produced a SO dysfunction model by vein injection of morphine in rabbit ear, and treated with Da cheng qi tang associates with magnesium sulfate to observe the effects on the sphincter of Oddi pressure and the changes of the NOS in local tissue.Objective: To investigate the method of building sphincter of Oddi disorder model in rabbits and effects of Da cheng qi tang,magnesium sulfate and the two combined on SO pressure and its possible mechanism, providing a reasonable theoretical basis on clinical treatment of SOD.Methods:1. Building sphincter of Oddi disorder mode in Rabbit: 12 New Zealand rabbits were randomly divided into 2 groups (normal group and experimental group), experimental group accepted ear vein injection of morphine hydrochloride. Observe SO pressure changes in two groups thr- ough SO manometry after 10 min, 20 min, 30 min to test whether SO dysfunction model was successful.2. The effects of different drugs on the SO pressure: 24 New Zealand rabbits were randomly divided into 4 groups (control group, magnesium sulfate group, Da cheng qi tang group and Da cheng qi tang + sulfate group), produced a SO dysfunction model by vein injection of morphine in rabbit ear, at the same time, saline, magnesium sulfate, Da cheng qi tang, Da cheng qi tang + magnesium were pumped into SO by micro-pump at constant speed ; SO pressure observe was obserbved through SO manometry 30 mins after administration3. Use immunohistochemical techniques to observe changes of NOS within the tissue of SO in each group 30 min after building mode. Results:1. 10 min, 20 min, 30 min after SOD compared with normal group in the same time point, SO basal pressure, contraction amplitude and contraction frequency in experimental group were significantly higher (P<0.01), indicating a successful SOD model.2. 30 min after SOD.SO contraction amplitude, contraction frequency in magnesium sulfate group were significantly lower than control group (P<0.01); SO basal pressure, contraction amplitude, contraction frequency in Da cheng qi tang group were significantly lower than control group (P<0.01),The effect of Da cheng qi tang associates with magnesium sulfate was the most obvious in all groups and the difference was significant.3. 30 min after SOD experimental groups NOS within the SO was less than normal group (P <0.01); in magnesium sulfate group and Da cheng qi tang group, NOS were significantly higher than the control group (P <0.01) while increase in Da cheng qi tang associates magnesium sulfate group was the most obvious.Conclusion:1.The SO dyskinesia model in morphine of this research is rapid and simple in performance, which can be used as a carrier of SOD research.2. Da cheng qi tang ,as well as magnesium sulfate ,can resist SO dyskinesia finduced by morphine, while combination of the two has the most obviously effective treatment,which can be used as clinical guidance.3. Da cheng qi tang,magnesium sulfate and the two combined may change diastolic function of SO through affecting content of inhibitory neurotransmitter, NOS, in SO. The specific mechanisms need further study.
