Objective: To investigate the image features and its clinical diagnostic value of cerebral vascular malformations by SWI (Susceptibility Weighted lmaging) ,that compared with the routine-MR scan.Methods:There are 80 patients with cerebral vascular malformations who were confirmed by clinical operation, DSA or MR(MRA,SWI et al). (45 male, 35 female , age range 3-89years, average age 49.63years). Including cerebral cavernous Hemangiomas(60 cases), venous malformations(20 cases), intracerebral capillary telangiectasia (3 cases),cerebral arteriove- nous malformation(5 cases) . The main manifestations included epilepsy (11 cases),headache and dizziness(43 cases),motor function and sensory disturbance(12 cases),barylalia(3 cases), visual disturbance (2 cases), discovered from physical examination(9 cases).course of disease from one day to 30 years. To use Signa HD 1.5T echospeed MRI unit,all patients were use MR scan,include axis and sagittal SE T1WI scan,axis or coronal T2WI scan and axis SWI scan,39 cases were use contrast enhancement scan. 25 cases were use 3D- TOF MRA scan and 8 cases 2 2D- TOF MRV.Scanning parameters: T1WI:TR 400 ms,TE 15 ms,slice thickness 6 mm,internal 1 mm(slice thickness and internal of T1WI were uniformed withT2WI);T2WI : TR 3000 ms,TE102ms;SWI TR 50ms,TE 40ms,slice thickness 2.0mm,internal 0mm,FOV:24cm,matrix:320×320,NEX:0.75, bandwidth: 31.25, flip angle 30°.3D- TOF MRA scan : TR 32ms,TE 15ms,flip angle 20°,bandwidth:15.63, matrix:256×192, NEX:1, FOV:20cm, slice thickness 1.6mm.overlap loc.:12.We should bolus inject Gd-DTPA through median cubital vein of 0.1mmol per kg,speed of 2.5 ml/s. Original data of SWI post processing should do in GE Advantage workstation 4.3 and obtain the corrected phase and reconstruction SWI imaging. tow high seniority radiologists compared the numbers of cerebral vascular malformations of blind method. The forms, ranges, signal charcteristic of cerebral vascular malformations SWI images were analyzed. Paired rank sum test were used on the number of lesions detective proportion.Paired t-test were used of lesions area between SWI and T2WI.There were significant difference with statistic analysis (P<0.05).Results:1,There are 316 focuses among 60 cases of CCH in SWI, in the study there is one peitient who have 75 lesions.In frontal lobe there were 47 cases,in parietal lobe 33,occopotal lobe 52,temporal lobe 74,basal ganglia 48,thalamus 22,pons 35,cerebellar hemisphere 16, T1WI can find 66 64 lesions(20.25%), T2WI can find 79 77 lesions(24.37%), the lesions of diameter under 1cm, T1WI can find 35 lesions(13.36%), T2WI can find 46 lesions(17.56%). SWI can find 262 lesions(100%).SWI can find more lesions and lager than routine-MR scan,and there are significant difference in detection rate of lesions (P <0.05). SWI can find renal vessels around lesions that cannot find in routine-MR scan,and also can find the larger range than routine-MR scan.2,Among 16 20 venous malformations, which accompany with 6 cases cavemous hemangioma, SWI,T1WI,T2WI can find medullary veins 103 lesions ( 100% ),24 lesions ( 23.30 % ),34 lesions ( 33.01 % )respectively.Draining veins 23 lesions(100%),16 lesions(69.57%),17 lesions(73.91%)。13 cases of contrast enhancement scan and SWI can find medullary veins 51 lesions ( 77.27 % ) and 66 lesions ( 100.0 % )respectively,draining veins 15 lesions(100.0%)。3,Frontal lobe were 3 cases, parietal lobe 3 ,temporal lobe 3, basal ganglia 1.SWI can find dilatation vein and drainage vein,can find same vessel number with contrast enhancement scan.one telangiectasis located in pons.5 cases of arteriovenous malformation , frontal lobe 1 case,parietal lobe 1,occopotal lobe 12,callosum 2 1 cases,both SWI can find more drainage veins thanand routine-MR scan find 5 pieces feeding arteries .SWI can find 7 pieces draining veins , routine-MR scan can find 4 piece.2 cases use MRA find 4 pieces feeding arteries,can't find draining veins.1 case use MRV find many draining veins.4,3 cases of telangiectasia have 17 lesions ,which accompany with 2 cases cavemous hemangioma,all lesions lied in pons, multiple spots and nodules.SWI is mainly hypointense, hyperintense in the center.Conclusion : SWI can show detective more slimmer lesions than routine-MR scan. It is predominant for cerebral vascular malformations, especially occult cerebrovascular malformation in figurate and quantitive diagnoses,and provide a new uninjury method. Contemporary,avoid the side reaction of contrast-medium. there are significant difference in detection rate of CCH.( P <0.05).It has high sensitive and special ,and has advantage of display small and slow flow veinvalue. in clinic apply.
