0bjective:To investigate the changes of soluble CD146 (s-CD146) and adiponectin in CKD5 patients without dialysis and continuous ambulatory peritoneal dialysis(CAPD) patients. To investigate their clinical significances in CKD5 patients. Methods:Three groups were collected: CKD5 patients without dialysis, continuous ambulatory peritoneal dialysis(CAPD) and normal controls. ELISA was used to examine soluble CD146 (s-CD146) and adiponectin in all. Other clinical parameters such as age, height, weight Body mass index(BMI), systolic blood pressure, diastolic blood pressure, Hb, Hepatic function, total cholesterol triglyceride, high density lipoprotein (HDL), low density lipoprotein (LDL), apolipoprotein, albumin, hypersensitive c reactive protein (hsCRP), creatinine, blood urea nitrogen(BUN), estimated glomerular filtration rate (e-GFR), Upro/Ucr, urinary protein of 24 hours. The changes of s-CD146 and adiponectin and other parameters among the three groups were observed and compared.Results:1. The level of plasma s-CD146 in CKD5 patients without dialysis and CAPD patients were significantly higher than the normal controls(P<0.01). The level of adiponectin in CKD5 patients without dialysis and CAPD patients were significantly higher than the normal controls(P<0.01).2. (1)The level of plasma s-CD146 in CKD5 patients without dialysis was positively correlated with Upro/Ucr, serum creatinine, systolic blood pressure(P<0.05), and was negatively correlated with Hb. There was no significant correlation between s-CD146 levels and other clinical parameters.(2)There was no significant correlation between plasma s-CD146 levels and the clinical parameters in CAPD patients.(3)In normal controls, plasma s-CD146 levels was positively correlated with age, systolic blood pressure, diastolic blood pressure(P<0.05), and was negatively correlated with e-GFR.There was no significant correlation between s-CD146 levels and other clinical parameters.3. (1)The level of plasma adiponectin in CKD5 patients without dialysis was positively correlated with total cholesterol, Upro/Ucr, urinary protein of 24 hours(P<0.05), and was negatively correlated with BMI, e-GFR and Hb, there was no significant correlation between adiponectin levels and other clinical parameters.(2)In CAPD patients, plasma adiponectin levels was positively correlated with diastolic blood pressure(P<0.05), and was negatively correlated with albumin. There was no significant correlation between plasma adiponectin levels and other clinical parameters.(3)In normal contrls, plasma adiponectin levels was positively correlated with age, diastolic blood pressure, HDL-cholesteral, creatinine (P<0.05), and was negatively correlated with e-GFR. There was no significant correlation between plasma adiponectin levels and other clinical parameters.4. In CAPD patients, the level of plasma s-CD146 was positively correlated with plasma adiponectin (P<0.05). In the normal contrls, the level of plasma s-CD146 was positively correlated with plasma adiponectin (P<0.01). In total patients, the level of plasma s-CD146 was positively correlated with plasma adiponectin (P<0.01). Conclusions:1. The levels of plasma s-CD146 and plasma adiponectin in CKD5 patients without dialysis and CAPD were significantly higher than the normal controls. They are believed to play important roles in chronic kidney disease.2. The level of plasma s-CD146 was correlated with some clinical parameters and plasma adiponectin.3. Speculating that s-CD146 may reflect the endothelial damage and vascular microinflammation aggravated in chronic kidney disease.
