Objective:To explore the relationship of PlGF and MMP-9 with vulnerable plaque, as well as to explore the relationship of PlGF and MMP-9 with short-term prognosis in patients with vulnerable plaque by PCI.Methods:110 cases were selected.There were 90 patients with coronary heart disease.It included 15 patients with ST-segment elevation myocardial infarction (STEMI), 15 patients with non-ST-segment elevation myocardial infarction (NSTEMI), 30 patients with unstable angina pectoris (UAP) and 30 patients with stable angina pectoris (SAP) .we selected 20 cases as the control group that had been excluded coronary heart disease by coronary angiography and other methods.This test is divided into five groups, namely, STEMI group, NSTEMI group, UAP group, SAP group and control group.3ml venous blood samples were collected in all cases, 3ml coronary blood samples were collected by catheter at coronary mouth in the patients with negative coronary angiography . Each 3ml blood samples were collected in the proximal and distal of the plaque by microcatheter before PCI(the patients with AMI after the opening of infarct-related artery) in the positive patients. The concentration of PlGF, MMP-9 were assessed by the method of ELISA.Results:1.In peripheral venous serum, coronary artery serum in the proximal of the plaque, coronary artery serum in the distal of the plaque, the Serum PlGF, MMP-9 levels of STEMI group, NSTEMI group, UAP group and SAP group were significantly higher than control group (P <0.01); the Serum PlGF, MMP-9 levels of STEMI group were significantly higher than NSTEMI group, UAP group and SAP group (P <0.01); the Serum PlGF, MMP-9 levels of NSTEMI group, UAP group were higher than SAP group (P < 0.05). 2. In the test , at the Serum PlGF, MMP-9 levels of STEMI group, NSTEMI group and UAP group, the distal serum of the plaque were higher than peripheral venous serum and the proximal serum of the plaque (P <0.05) . At the serum PlGF, MMP-9 levels of SAP groups, the distal serum of the plaque ,peripheral venous serum and the proximal serum of the plaque were no significant difference (P> 0.05).3.In the 30-day follow-up period, the incidence of adverse cardiovascular events was increased while the PlGF concentration were increased.4.In the 30-day follow-up period, the incidence of adverse cardiovascular events was significantly increased in the group of high MMP-9 concentration.Conclusion:1. Vulnerable plaque may promote the PlGF and MMP-9 secreting, stability plaque secrets smaller than it.2.The PlGF and MMP-9 are closely related to plaque instability.3.The serum levels of PlGF are related to the incidence of recent adverse cardiovascular events in patients with vulnerable plaque after PCI ,and that have concentration-dependent trend.4.The incidence of recent adverse cardiovascular events is increased in patients with vulnerable plaque after PCI that have high concentrations of serum MMP-9...
