Purpose of the study: Porous tantalum screw insertion in the femoral head is a new treatment for the early stage ANFH, while, quadratus femoris muscle-pedicle bone graft transplantation is a traditional hip preserve procedure. This study aim to compare the two surgiacl treatment on the aspect of blood lossing, operation time and therapeutic effect.Methods: We set up two surgical group, the tantalum screw insertion group and the muscle-pedicle bone graft transplantation group. Both groups have the same selected standard, non-traumatic ANFH at early stage(Ficat stage II). the tantalum screw insertion group include 10 cases(14hips), male 9 cases, female 1 case, mean aged 38.7 (32-50) years, who all underwent surgery during the period of 2008. 3-2008. 12. The muscle-pedicle bone graft transplantation group include 21cases(27hips), male 20 cases, female 1 case, mean aged 36. 4 (26-60) years who all underwent surgery during the period of 2007.7-2008.8 . we collect the surgical information, blood loss and operation time. Also we evaluate the hip function with Harris score system before and after operation. meanwhile, we follow up the X-ray image evaluate the lasting time before femoral head collapse.Rensults: Comparing with the muscle-pedicle bone graft transplantation group , the tantalum screw insertion group has a less blood loss and operation time (P<0.01) . Both groups have no difference in the pre-operation Harris score (p=0.561) . The tantalum screw insertion group have a superior post-operation Harris score (p=0.009) . We followed up in both groups, average 8.5 (4-12) months for tantalum screw insertion group, and average 11.4 (7-20) months for muscle-pedicle bone graft transplantation group. Kaplan-Meier analysis at 8.5 months resulted in an 100% and a 96. 3% survivorship for the tantalum implant and bone graft transplantation, respectively. There was not a statistical difference in survival at 8. 5 months of follow-up.Conclusion:Comparing with the muscle-pedicle bone graft transplantation, the tantalum screw insertion is a mini-invasive procedure, it take less time for the patient to recover, and the short term therapeutic effect is encouraging. But, the functon of prolonging the head collapse is not very obvious, the final therapeutic effect for the tantalum screw insertion should not be determined before more long term and large amount follow up study.
