Study On The Relationship Between The Expression Of CD44v6 In Early Gastric Cancer And Micrometastasis Of The Lymphnodes

[ABSTRACT]:Objective:To detect the expression of CD44v6 in early gastric cancer and micrometastasis of the lymphnodes,as well as the correlation between the two,and to deduce its clinical significance.[METHODS]:From From February 2004 to December 2008 period, 64 cases of early gastric cancer patients with surgical resection in the Qilu Hospital of Shandong University who do radical surgery of gastric cancer and(or) lymph node dissection,whose primary tumor with immunohistochemical CD44v6 to detect,the lymph nodes that conventional HE staining were negative carry out with CK20 immunohistochemistry.Combined the clinical and pathological indicators,expression of CD44v6 have significant correlation with lymph node micrometastasis,tumor diameter, it' s histological type and depth are also evaluated.The relationship between the expression of CD44v6 and lymph node micrometastasis is comprehensively analysed and evaluated in this research.[RESULT]:Out of 64 patients with early gastric cancer,the expression of CD44v6 by immunohistochemical staining is noted, Result was 23 cases found positive,positive rate is 35.94%,Out of which 10 cases showed focal positive.31 cases were negative. Out of 64 cases of EGC patients,the total of negative HE staining is 782 lymph nodes,while using CK20 immunohistochemical staining of six cases,the 14 lymph nodes have cancer cells,also shows the existence of lymphatic sinus of a single cancer cell that stained positive for CK.All specimens in this group of lymph node micrometastasis are the lymph nodes located in the first station and second station The incidence rate of micrometastasis in patients is 9.38%(6/64),lymph node metastasis rate is 1.79%(14/782).Out of 6 cases of lymph node micrometastasis,the five cases have the largest diameter＞2.0cm size tumors(83.33%;),the incidence of micrometastasis is 17.24%(5/29),only 1 case have the diameter＜2.0cm size tumor (16.67%),the incidence of micrometastasis to 2.86%(1/35), x 2 test P＜0.05;histological types are the well-differentiated and differentiated adenocarcinoma was found in micro-metastasis. Four cases were of poorly differentiated adenocarcinoma (66.67%),the incidence of micrometastasis is 21.05%(4/19), two cases of signet-ring cell carcinoma(33.33%),the incidence of micrometastasis is 25.00%(2 ),x~2 test P＜0.05;with tumor invasion between(M～SM),lymph node micrometastasis in cases 0,1,5,respectively,with respective of 0,16.67%and 83.33%. The incidence of micro-metastasis(0,0 / 16),(4.55%,1 / 22 ), (19.23%,5 / 26)respectively,x~2test P＜0.05.CD44v6 staining primary tumor,tumor diameter＞2.0cm with positive rate of 37.14% (13/35),tumor diameter＜2.0cm-with positive rate of 34.48% (10/29),x~2 test P＞0.05;histological types are well-differentiated adenocarcinoma with positive rate of 35%(7 / 20),differentiated adenocarcinoma with positive rate 29.41% (5 / 17),poorly differentiated adenocarcinoma with positive rate of 42.11%(8 / 19),signet-ring cell carcinoma with positive rates of 37.50%(3 / 8),x~2 test P＞0.05;Regarding the tumor invasion (M～SM),the positive rates were(18.75%,3/16),(22.73%,5 / 22),(57.69%,15 / 26) respectively,x~2 test P＜0.05.Out of 23 cases with positive for CD44v6 expression,the four cases (17.39%) have micro-metastasis,Out of 31 cases of patients with negative expression of CD44v6,the 2 cases(6.45%)have Micrometastasis,x~2test P＜0.05.[CONCLUSION]:This set of data shows that lymph node micrometastasis have significant correlation with tumor diameter, histological type and depth of invasion(P＜0.05).CD44v6 positive expression have no significant correlation with tumor diameter and histological type(P＞0.05),but with violations related to a reduction of the depth(P＜0.05).The deeper the tumor invasion, the more positive expression of CD44v6.The expression of CD44v6-positive and lymph node micrometastasis have a significant correlation(P＜0.05).
