ObjectiveTo study on the pathological change and effect about HBVcccDNA of hepatic tissue after treatment with Adefovir dipivoxil in chronic hepatitis B.MethodsWe screened 20 chronic hepatitis B patients (including 11 men and 9women) aged 17-45 years old (average 33) who were admitted to Fengtian Hospital affiliated to shenyang medical college between May 2007 and May 2009. These patients were treated with adefovir 10mg per day orally and the course of treatment was more than 48 week. Quantitative PCR were used to detect HBVcccDNA in liver biopsies before treatment and at 48w. The same method was used to examine HBVDNA in sera and liver function, Liver tissue inflammatory activity grading (G) and degree of fibrosis staging (S) were evaluated.ResultsHepatic pathological ameliorative rate at 48w of treatment by adefovir is 75%; 70% cases got absolute viral response at 48w of treatment by adefovir; HBVcccDNA level in hepatic tissue decreased 0.91og at 48w; There was a positive correlation between the cccDNA level in the tissue and HBVDNA level in the sera(r=0.496,p<0.05); There was no correlation between quantification of hepatic tissue HBVcccDNA and the degree of liver parenchyma injury and hepatic tissue pathology.Conclusions1. Adefovir can improve or delay the development of pathological change in the patients of chronic hepatitis B. 2. Adefovir can depress the level of HBVcccDNA in hepatic tissue.3. There was a positive correlation between the HBVcccDNA level in the tissue and HBVDNA level in the sera.
