Background:Our country has been in an unprecedented period that our nuclear power is developing greatly. It's depend on the public acceptability and safety that the sustainable development of nuclear power. Now many surveies in the resident near the nuclear installations recognized that there was excess risk of leukemia especially children round the nuclear installations.It had been proved in the life-study of REFE that:the incidence of leukemia in the people who accepted low dose of radiation were rasied.There were an important adjustment in our country that moderate progress in nuclear energy industry was put forward in 2003.The nuclear energy project in our country has got enough progress.There had been concerned about incidence in cancer near nuclear installations especially leukemia.We have developed a lot of health investigations about residents around the nuclear power station,such as Qinshan, Da-ya-wan and Tianwan nuclear power station et al.All the survey admit the running of the power station didn't cause effects to resident.Because of the smaller populations and leukeamia cases,e.g there were 11.63 residents around 20km of Dayawan NPP,there were no deeply investigations.But there are no relative report about surveies to the resident around nuclear installations except power station.Therefore it is necessary to carry out surveies about malignant especially the condition of leukemia in the population around the nuclear installations,and grip the information about the health of population. Our study is the first survey about health of resident near the making of nuclear element and uranium concentrate factory,which could accumulate treasure data and experience for the health survey about population near the nuclear installations.Objectives:To investigate the prevalence and the trend of leukaemia among people living around 202 region.To compare the incidence of leukaemia with the people living in Baotou city and Kundulun region.To predominate the baseline informations about residences around the 202 region. Methods:The investigate site is 202 region which is a factory of nucleus installation in Baotou city in Inner Mongolia.The case has been living inside 10Kin around 202 region and the control has been lliving outside the 10Kin around 202 region.The study project was been worded out based on the research population and geography situation.We selected the medical institutions and chose the staff who worked in the investigated hospital or the local Center for Disease Control and Prevention.Leukemia was diagnosed by marrow test.In order to collect all of the new leukemia cases between 1997 to 2007,we investigated all medical institutions in Baotou and provincial-level medical institutions in Inner Mongolia From March 2008 to March 2009,meanwhile we completed the case report card and screened the case respectively.After the initial cases classification,we check the information of reported cases with the police station,funeral parlor and community.Acording to their living place,we categorised the case intotwo groups,then input information to database.The investigated communities were divided into the urban and rural places.In collaboration with community staff we investigate the leukemia prevalence between 1997 and 2007 in the investigated community,personnel fill in "community review of leukemia cases questionnaire" and check the cases by the copied cases collected in medical institutions,the possible missed diagnosis or treatment situation were being found.Copying the social-demographic characteristics from the Bureau of Statistics and the Public Security Bureau between 1997 and 2007 in Baotou.To estimate the demographic characteristics and the leukemia prevalence of people living inside 10Km around 202 regionResults:1.The analysis of the leukemia incidence for the whole population in Baotou city.(1) The survey indicates that there are 643 new leukemia cases for the whole population in Baotou city during 1997 and 2007.The crude leukemia incidence rate is 2.83/100,000(male 3.13/100,000,female 2.52/100,000),and the normalized incidence rate in China is 2.85/100,000(male 3.07/100,000,female 2.57/100,000). The incidence rate in male is a little higher than that in female. (2) The leukemia incidence rate for the whole population in Baotou city gradually increases over time during 1997 and 2007.The incidence rate varies by sex.The leukemia incidence rate statistically increases in male(X~2=3.959,P=0.042),but stable in female(X~2=0.063,P=0.813).(3) The 296 cases of AML got the highest proportion in various leukemia(46.03%), followed by 167 cases of ALL(25.97%),108 cases of CML(16.80%),29 cases of CLL(4.51%),and other kinds(6.69%).(4) The leukemia age-incidence rate varies slightly with the age before 35 years old, and increases gradually but slowly after 40 years old.There is a pinnacle after 65 years old which don't exist in children group under 14 years old.The age-incidence rate of kinds of leukemia has various tends over age.The incidence rate of ALL exists a pinnacle for children under 10 years old and gets a high incidence rate in group older than 65 years old.And other kind of leukemia is rarely seen in children. AML forms a pinnacle in the period of 65-75 years old,CML in the period of 65-70 years old.There is only one case of CLL in group under 30 years old and incidence rate of CLL increases after 60 years old.2.The situation of leukemia incidence in 10km rang around 202-Factory and the comparison with in Baotou city and other areas.(1) The survey indicates that there are 236 new leukemia cases(126 cases for male,110 cases for female) in the investigation zone during 1997 to 2007.The crude leukemia incidence rate is 2.51/100,000(male 2.63/100,000,female 2.39/100,000),and the normalized incidence rate is 2.53/100,000(male 2.65/100,000,female 2.43/100,000).Either male or female incidence rate in the investigation zone is lower than that in whole city and Kun district.(2) The leukemia incidence rate in investigation has no significant changes over time during 1997 to 2007(X~2=1.858,P=0.185).Either male or female incidence rate has no significant changes over time(male X~2=3.959,P=0.042,female X~2=0.283, P=0.617).(3) The sequence of proportion of leukemia incidence rate is AML(44.91%),ALL (26.27%),CML(19.49%),other specific and undefined cell type leukemia(6.69%), and CLL(2.97%).(4) The leukemia incidence rate of children under 14 years old in investigation zone is 2.04/100,000(whole city 2.09/100,000).The one in 0-4 years old group in investigation zone is 2.73/100,000(whole city 2.07/100,000),and in 10-14 years old group in investigation zone is 1.30/100,000(whole city 2.05/100,000).Conclusions:1.It had reported that the incidence of leukemia in our country is about 3/100,000.This survey results shows that the incidence of leukemia,sex,the types of leukemia and the aged-incidence are consistent with the other corresponding investigartions in our country.2.The incidence of leukemia around 202 region is the general level in our country.The incidence of leukemia around 202 region is lower than that in Baotou and Kunqu. There are no trends about incidence.The types of leukemia is the same with that in Baotou.The incidence increases with age.The peak appears in the oldman.The leukemia prevalence of 14 years age old age in this survey is lower than in the same age of Baotou City.We do not found that the incidence of leukemia around 202 region is higher than that of Baotoucity.
