Expression Of ERCC1 Predicts The Survival And Correlates With The Response Rate Treated With Platinum-based Chemotherapy In Esophageal Squamous Cell Cancer

Esophagus carcinoma is the eighth common carcinoma of mortality all over the world,especially higher in China.And almost 95%of them are esophageal squamous cell cancer(ESCC).Although we know the pathologic types,TNM stage,treatment pattern,molecular marker and the general state of health of patients are the prognostic factors of ESCC,but the special molecular marker is still unknown.So we analyse the prognostic factors of the ESCC and investigate the correlation between ERCC1 and the response rate of advanced ESCC treated with platinum-based chemotherapy.We retrospectively reviwed the 114 patients with ESCC who underwent treatment in Cancer Hospital of Chinese Academy of Medical Sciences from 1991 to Des 2008,and got the Formalin-fixed,paraffin-embedded biopsies of those 114 patients.The expression of ERCC1 were assessed by immunohistochemistry.The expression of ERCC1 and correlates with the response rate treated with platinum -based chemotherapy in adcanced ESCC,and other clinical and pathological characteristics,survival and prognostic factors were analyzed by SPSS 13.0 statistic software.We got the results:53(46.5%) patients were ERCC1 positive and 61 (53.5%) were negative.The median survival of the 114 patints was 24 months,median DFS of the 92 patints was 10 months.By the single-variant's Kaplan-Merier analysis,the tumor staging,ERCC1 expression,chemotherapy,degree of differentiation,smoking,BMI and number of LN metastasis were associated with the poor OS,after the multi-variants' Cox regression analysis,we found the tumor staging was the independent prognostic factors,P value were 0.038.By the same way, single-variant's Kaplan-Merier analysis the DFS:the tumor staging,ERCC1 expression,ECOG score,LVSI and the number of LN metastasis were associated with the poor DFS,after the multi -variants' Cox regression analysis,tumor staging and ECOG score were the independent prognostic factors,P were 0.039,0.045, respectively.By the same way,single-variant's Kaplan-Merier analysis the PFS:the response rate,chemotherapy cycles,full-dose of cisplatin,sex and smoking were associated with PFS,after the multi-variants' Cox regression analysis,the response rate and full-dose of cisplatin were the independent prognostic factors,P value were 0.000,0.037,respectively.There was no statistical correlation between ERCC1 and the response rate in advanced ESCC.,but in sub-group patients with full-dose cisplatin,P value was 0.031.In conclusion,this retrospective study showed that positive ERCC1 expression maybe an independent prognostic factor for OS and a signal of resistance to cisplatin.
