Objective: To investigate the protective effect of intraperitoneal resuscitation on small intestinal mucosa in rats with hemorrhagic shock.Methods: 50 Sprague-Dawley (SD)rats were randomly divided into five groups, including sham operation group (groupâ… ), hemorrhagic shock group (groupâ…¡),intravenous resuscitation group (groupâ…¢), intravenous resuscitation plus saline intraperitoneal resuscitation group (groupâ…£),intravenous resuscitation plus PD-2 solution intraperitoneal resuscitation group(groupâ…¤), 10 rats in each group. The rats of groupâ… were successively treated with right common carotid artery catheterization, right femoral vein catheterization, left femoral arterial catheterization and whole body heparinization. The rats model of hemorrhagic shock was produced according to Wigger's method in groupâ…¡,MAP was maintained at 40 mmHg for 60 min by femoral artery on the basis of groupâ… . The rats of groupâ…¢were resuscitated with shed blood plus 2 equal volumes of Ringer's solution on the basis of groupâ…¡. The rats of groupâ…£were injected for intraperitoneal therapy with 30 ml saline on the basis of groupâ…¢. The rats of groupâ…¤were injected for intraperitoneal therapy with 30 ml PD-2 solution on the basis of groupâ…¢. The materials of blood and small intestine in groupâ… were drawn after 120 min by right common carotid artery catheterization, right femoral vein catheterization, left femoral arterial catheterization and whole body heparinization in rats continuously. The materials of that in groupâ…¡were drawn after 60 min by the mold-building in rats. The materials of that in groupâ…¢,â…£andâ…¤were drawn After 60 min by resuscitation .To determine activity of plasma diamine oxidase(DAO) by chromatometry, content of D-lactate(D-LA) by spectrophotometry and endotoxin by nephelometry ,and observe histopathological and morphological changes of small intestinal in rats with light microscope and electronic microscope.Results: There were a remarkable difference in activity of DAO, content of D-LA and endotoxin in rats between that of groupâ…¡and groupâ… (P<0.01), groupâ…£and groupâ…¡(P<0.05) , groupâ…¤and groupâ…¡, III and IV (P<0.05 or P<0.01) . There was no statistical significance of activity of DAO, content of endotoxin between groupâ…¢and groupâ…¡(P>0.05) except content of endotoxin D-LA (P<0.05) .The pathomorphology changes of small intestinal were observed with light microscopy in rats. It was found that pathomorphology changes of small intestinal were presented with severe edema, patchy necrosis, shedding, defect of mucosa, collapse of submucosal vascular, and focal necrosis of intestinal lamina propria gland. Changes of small intestinal in groupâ…¢andâ…£were slightly in compared with that of groupâ…¡. Changes of small intestinal in rats of groupâ…¤were presented with mild edema and lodging floss,moderate filling of submucosal vascular. The sub-cellular morphology changes of small intestinal mucosa were observed with electron microscopy in rats. It was found that small intestinal mucosa in rats were high degree of swelling of mitochondria, loss of cristae, swelling of endoplasmic reticulum or integration, sparse microvilli epithelial cells, a large number of condensed nuclei, chromatin margination. Changes of small intestinal mucosa in rats of groupâ…¢andâ…£that were mitochondria, endoplasmic reticulum, microvilli, condensed nuclei, chromatin margination in rats were slightly in compared with that of groupâ…¡.Small intestinals in rats of groupâ…¤were minor swelling of endoplasmic reticulum and epithelial cell mitochondria, still structured of microvilli epithelial cells.Conclusions: 1. Rats model of hemorrhagic shock was established successfully according to Wigger's method. 2. Intravenous resuscitation plus PD-2 solution intraperitoneal resuscitation could significantly reduce plasma DAO activity, content of D-LA and endotoxin,suggesting a protective effect to small intestinal in HS rats by improving the changes of histology histopathology and sub-cellular morphological and inhibition of translocation of intestinal flora. 3. The protective effect on small intestinal mucosa by PD-2 intraperitoneal fluid resuscitation in hemorrhagic shock rats might be better than that of the intraperitoneal resuscitation by normal saline.
