Objective:To detection of premature rupture of membranes(PROM) by maternal serum Interleukin-18(IL-18) levels and fetal membranes in the expression of matrix metallopro-teinase-2(MMP-2), of both the premature rupture of membranes occurred in the development of the role and relevance, to explore the predicti-on the feasibility of new methods of premature rupture of membranes, premature rupture of membranes provide a theoretical basis for the prevention and treat-ment.Methods:Selected pregnant women with preterm premature rupture of membranes (pPROM), term premature rupture of membranes(tPROM) of pregnant women, pregnant women in term fetal membranes have not broken, all 25 patients as research subjects were divided into preterm premature rupture of membranes group (pPROM group), term premature rupture of membranes group (tPROM group), full-term fetal membrane rupture group (control group).All subjects were taken in the not in labor before the elbow vein 3ml, using solid-phase sandwich enzyme-linked immunosorbent assay (ELISA method) were detected by maternal serum levels of IL-18;All subjects were taken immediately after birth, fetal membranes fetal membranes break Department, using hematoxylin eosin (HE) staining in all subjects of the fetal membranes for subclinical chorioamnionitis diagnosis; by immunohistochemical histochemical staining, and Nikon of Japan's Nikon ECLIPSE 50i microscope image acquisition system and MiVnt image analysis system to determine color in the cytoplasm of the brown positive MMP-2 protein expression in gray-scale objects.These experimental data was analized by the software SPSS11.3.0Results:1. the groups in maternal serum levels of IL-18 Comparison:premature rupture of membranes group 57.47±4.83 ng/mL, term premature rupture of membranes group 48.72±3.77 ng/mL, full-term fetal membrane rupture group 40.32±4.42 ng/mL. Analysis of variance, F= 96.47, p<0.001, difference, the difference was statistically significant. Pairwise comparison further revealed that each of the two groups in serum levels of IL-18 were different, the difference was statistically significant (P<0.001). PPROM IL-18 serum levels in the highest, followed by premature rupture of membranes group, the lowest full-term fetal membrane rupture group.2. Postpartum fetal membranes of pregnant women in each group in the expression of MMP-2 gray-scale comparison:premature rupture of membranes group 128.58±2.91, premature rupture of membranes group 159.46±3.03, full-term fetal membrane rupture group 195.12±3.04. Analysis of variance, F= 3087.76, p<0.001, difference, the difference was statistically significant. Pairwise comparison further revealed that each of the two groups in fetal membranes of the expression of MMP-2 were different gray levels, the difference was statistically significant (P<0.001). PPROM group the lowest gray level, followed by premature rupture of membranes group, the highest full-term fetal membrane rupture group. According to the lower gray level, said the higher the positive expression, the higher gray-scale expression, the lower the diagnostic standard, the fetal membranes preterm premature rupture of membranes group MMP-2, the minimum gray level, the positive expression of the most significant.3. subclinical chorioamnionitis group and the non-chorioamnionitis group of maternal serum levels of IL-18 Comparison:subclinical chorioamnionitis in IL-18 serum levels (56.73±5.17ng/mL), non-human chorionic sheep meningitis in IL-18 serum levels (48.83±4.19ng/mL), two groups t test found that, t= 5.87, p <0.001, difference, difference was statistically significant. Subclinical chorioamnionitis in serum levels of IL-18 was significantly higher than non-chorioamnionitis group.4. subclinical chorioamnionitis group and the non-chorioamnionitis group MMP-2 expression in fetal membranes in comparison:subclinical chorioamnionitis and fetal membranes in the MMP-2 in the gray level (134.39±12.57), non-fluff membrane fetal membrane in sheep meningitis MMP-2 in the gray level (155.33±2.34), t-test showed the two groups, t=-6.16, p<0.001, difference, the difference was statistically significant. Subclinical chorioamnionitis and fetal membranes in the gray-level MMP-2 was significantly lower than non-chorioamnionitis group.5. on the PPROM group and premature rupture of membranes in serum levels of IL-18 and MMP-2 in fetal membranes of gray-scale correlation analysis showed that serum IL-two 18 levels and fetal membranes in the MMP-2 in gray scale, negative correlation, the correlation coefficient r=-8.5, P<0.001. Based on the correlation coefficient r=-8.5 deduced closely related to higher levels. As the fetal membranes, MMP-2, the gray scale inversely with the expression, gray scale, the lower the higher the positive expression, that PROM maternal serum levels of IL-18 and fetal membranes in the positive expression of MMP-2 positively correlated.Conclution:1.Maternal serum levels of IL-18 increased the incidence of premature rupture of membranes may be involved.2.Merge subclinical chorioamnionitis in serum of pregnant women with PPROM group in higher levels of IL-18, suggesting that IL-18 is not only involved in infection caused by premature rupture of membranes more closely related with pregnancy.3.Maternal fetal membranes increased expression of MMP-2 may be involved in the occurrence of premature rupture of membranes.4.PPROM group of maternal fetal membranes was higher than MMP-2expression in premature rupture of membranes group, suggesting that fetal membranes in the expr ession of MMP-2 by serum IL-18 regulation. 5.Subclinical chorioamnionitis, and serum levels of IL-18 is closely related to inc-reased serum levels of IL-18 increased as the clinical prediction of subclinical chorio-amnionitis one of the indicators6.Subclinical chorioamnionitis, and fetal membranes increased expression of MM P-2 are closely related to fetal membranes increased expression of MMP-2 may be used for clinical diagnosis of subclinical chorioamnionitis one of the indicators.7.Premature rupture of membranes in pregnant women with elevated levels of IL-18 and fetal membranes increased expression of MMP-2 was positively related, indica-ting the occurrence of premature rupture of membranes in the course of serum levels of IL-18 level may be involved fetal membranes in the expression of MMP-2 positive adjustment, both the incidence of premature rupture of membranes involved in collaboration..
