Premature rupture of membranes is obstetrics complication which can threaten mother and infant common health. The frequency of premature birth has actually increased over the past 3 decades, which in part can be attributed to the fact that the exact cause of these conditions is still unknown. The incidence of preterm premature rupture of human fetal membranes ( PROM) is 12. 4% , of which before term(PPROM) is 2 -3. 5% and accounts for 30 -40% of all preterm deliveries. After PROM, the complications, such as infection, premature birth, prolapsse of cord , placental abruption, amm'otic embolism, fetal distress, great increases the perinatal morbidity and mortality rate.Matrix metalloproteinase 9 ( MMP-9) is a member of the MMP-9 gene family that encodes zinc - dependent enzymes that break down extracellular matrix (ECM) macromolecules. MMP-9 degrades type IV collage, which is a .major compent a basement membranes of amniochorion membranes, denatured fibrillar collagen, and other ECM molecules including laminin and fibronectin. Amnion eqithelia cells produce MMP-9 and induction of MMP-9 in this layer promotes degradation of the underlying basement membranes leading to cell detachment and apoptotic death. The increased expression of MMP-9 in membranes from the PROM group showed The goal is that MMP-9 is associated with premature rupture of fetal membrane but is not associated with preterm labor.The immunohistochemical (streptavidin - perioxidas) assay was performed to study the expression of the MMP-9 during premature rupture of fetal membranes and preterm labor with intact membranes and term labor after vaginal delivery , which may further express the roles of MMP - 9 in the premature rupture of membranes, which may provide the possibility of preventing the premature rupture of membranes by suing the issue inhibitor of metalloproteinase, thus de-creased the comlication of the premature rupture of membranes .MaterialsAmniochorion samples were collected from 2 groups of women, women with premature rupture of membranes ( n = 30) and women with preterm labor with no rupture of membranes (n = 16) and women with term labor with no rupture of membranes (n = 10). women with uncomplicated, healthy signleion term pregnancies China Medical University affiliated the number 2 Hospital in Shenyang between January 2003 and December 2003 were invited to participate, We included those with conditions, including history of non - smoking, vaginal delivery , no antibiotic treatment. Immunohistochemical analysis was employed to determine the level of matrix metalloproteinase - 9 in fetal membranes of premature rupture of membranes and premature labor and term labor. After paraffin sections were blocked endogenetic peroxidase by 0.3% hydroper-oxide, antigen plerosis by heat, added MMP-9 polyclonal antibody. Then they were added to a series of antibodies. At last they were stained by DAB. Brown and yellow granulae emerging in cytoplasm defined as positive expression, and analyzed by pathologic image analysis system. Data were analyzed to determine statistical significance by using student t test.ResultsImmunohistochemical analysis results demonstrated an increased MMP - 9 protein expression for in prematurely ruptured membranes compared with preterm Labor membranes, MMP - 9 protein is brown and yellow granulae emerging in cytoplasm defined as positive expression.
