Object:The purpose of this study is to investigate the effect of thymosinα-1(Tα-1) to inflammatory factors and immune function after children cardiopulmonary bypass(CPB) and to discuss the function of thymosinα-1.Patients and method:From July to December 2009,forty patients who were scheduled for heart defects correction were randomly divided into two groups,which contain twenty patients.In the experimental group(E group) thymosinα-1 which the dose of was 0.9mg/m~2 were injected subcutaneously in the perioperative period,which contain 3 days before and 3-5 days after surgery.And in the control group(C group) thymosinα-1 were not injected subcutaneously.The artery blood samples were collected to measure the plasma concentration of IL-2,IL-4,IL6,IL8,IL-10,TNF,IFN-γ,CD3+,CD4+,CD8+ and CD4+/CD8+ when at the onset of being hospitalized(T0),CPB finish(T1),2h after CPB (T2),24h after CPB(T3) and 5d after CPB.Time of free from anesthesia,ICU, antibiotic use and hospital were also recorded. Results:There were no significant differences between the two groups of mean age,mean weight,ejection fraction,the duration of mechanical ventilation and ICU settling,mean aortic cross-clamp duration or mean bypass duration(P>0.05).But it was not in the time of antibiotic using(P<0.05).None of the patients died and had severe infect.There was no arrhythmia in each group.The concentration of IL-2,IL4,TNF,IFN-γwas not obviously changed whenever before and after CPB,and was not in the E group and C group.But the concentration of IL-6 and IL-10 was significantly changed.IL-6 went up sharply at T1 and descended gradually after T1 in both E group and C group,but at T1 that went up more sharply in C group(P<0.05).Similarly,IL-10 ascended quickly after CPB and arrived its peak at T2,and at T1 that had significant difference between the experimental and the control group(P<0.05).Both CD3+ and CD4+ declined after CPB, and ascended at T4,and that in E group at T4 that was higher than in C group(P<0.05). The ratio of CD4+/CD8+ descended in C group at T1 and T3(P<0.01),on the contrary,in E group that had not changed at T1 and T3.At T4,the ratio were higher in E group(P<0.05).Conclusion:1.Thymosinα-1 can inhibit the release of IL-6 and promote the release of IL-10, which can reduce inflammation after cardiopulmonary bypass.2.Thymosinα-1 can obviously boost cell immune function,which contains CD3+, CD4+,and increase the ratio of CD4+/CD8+to reduce severe infect.
