BackgroundColorectal cancer is one of the most common malignant tumor in our country In recent years,along with lifestyle such as diet and living environment that changes in our country,especially in big cities,the incidence of colorectal cancer showed an upward trend year by year,and has become the most common,the incidence rate of the fastest growing one malignant tumor,and in the future will continue to steadily rising.Due to the delitescence and slowly development in the early stage,as well as the relative delayed metastasis,most of the initial symptoms of colorectal carcinoma are ignored.As long as the bowel obstruction,abdominal mass or distant metastasis are found,the advanced stage will be confirmed.Deadly disease of cancer,not only is the growthing out of control of cancer cells,even in its metastasis.It can be say,it only metastasis makes cancer so dangerous.Therefore,in order to obtain earlier diagnosis and prolong the survival,searching for some valuable pathological markers which indicate the disease progression and prognosis appears more and more imperative.Extracellular matrix and basement membrane play an important role in tumor invasion and metastasis.Tumor invasion and metastasis must first breakthrough extracellular matrix and basement membrane.This is the biochemical basis of tumor cells infiltration to the surrounding.Therefore,it has been that the matrix surrounding cancer cells and normal extracellular matrix should be differentiated,one important change is tenascin over expression appeared in the matrix around the cancer cells.Tenascin is an extracellular matrix glycoprotein,with promotion and regulation functions to variety of cells,such as cell adhesion,signal processing etc,have extensive interactions with other extracellular matrix molecules,plays an important role in the process of tumor invasion.In vitro study found tenascin have the role to promote invasion potential of tumor cells.Tenascin-W is a new member of the tenascin family,in the embryonic period,a variety of tissues are highly expressed,in some mature organizations,also a low expression,Ont he Tenascin-W expression in colorectal cancer tissues,foreign hasfew reports,has not been reported in china.Purpose1,In this study,detect tenascin-w expression in normal colorectal tissues,colorectal adenoma,ulcerative colitis,colorectal cancer organization by immunohistochemistry method,investigate the differences expression of tenascin-w in the above tissues.2,Investigate the expression of Tenascin-w in colorectal cancer,relationship with clinicopathologic factors and the clinical significanceMethodCollected pathology speciments preserved paraffin 78 cases of colorectal cancer from Second Affiliated of Kunming Madical College in July 2008 to November 2009,and statistical of patients with clinical and pathological information.In patients with clinical and pathological information includes the following items:general situation, including age,gender;tumor characteristics including tumor size,histological grade, lymph node metastasis,Ducks phases.Over the same period 30 cases of paraffin-embedded specimens of adenomatous polyps,43 cases of paraffin-embedded specimens of ulcerative colitis,14 normal mucosa specimens Using immunohistochemical SP method to determination 78 cases of colorectal cancer,30 cases of adenomatous polyps,43cases of ulcerative colitis and 14 cases of normal colorectal tissue expression of Tenascin-w.Statistical analysis of the relationship between the expression of Tenascin-w and the clinicopathological characteristics of colorectal cancer.Results1.In all cases,the positive expression of Tenascin-w located in the intestinal stromal and glandular cells cytoplasm,to yellow to brown granules.Tenascin-w expression in colorectal carcinoma, located in the matrix around the cancer nests,cytoplasm of tumor cells. Tenascin-w expression in the matrix around the cancer nests was break-like,continuous linear,mesh brown color for the matrix,The positive expressed in cell was brownish yellow color in cytoplasm.2.Histologically normal mucosa shows negative staining of tenasin-w.3.Tenascin-w expression between the colorectal cancer and the ulcerative colitis was no significant difference(P=0.034>0.0167),Tenascin-w expression between the colorectal cancer and the adenomatous polyps was no significant difference(P=0.874>0.0167),Tenascin-w expression between the ulcerative colitis and the adenomatous polyp was no significant difference(P=0.123>0.0167),Tenascin-w expression in the three tissues no significant difference.4.The expression of tenascin-w in each group of colorectal adenomas with mild, moderate and severe dysplasia were no significant difference.(P>0.0167)5.According to the colorectal cancer differentiation,depth of invasion,lymph node metastasis,distant metastasis,Ducks stage clinicopathological parameters in different stages,Tenascin-w positive expression were significantly different,but the tumor sizeConclusion1.Histologically normal mucosa shows negative staining of tenasin-w.2.Tenascin-w expression in colorectal cancer,colorectal adenomatous polyps, ulcerative colitis was no significant difference.3.Tenascin-w expression in colorectal cancer tissues has nothing to do with sex,age, tumor size,and relationship with the degree of tumor differentiation,depth of invasion,lymph node metastasis,distant metastasis and staging Ducks,Can be used as biological indicators for evaluating colorectal cancer invasion and metastasis.4.The Tenascin-w expression in colorectal adenomas has not related to organizations dysplasia. 5.Tenascin-w expression level of colorectal cancer patients as a prognostic indicator.
