The hepatorenal syndrome(HRS) is the severe complication of the patients who have the chronic hepatic disease and advanced stage hepatic failure. The main character of the HRS is kidney function failure, the marked difference of the artery circulate, the activity system active of the vascular and the reduction of the glomerulus's filtration rate. The main expression of the HRS is the progressive oliguria and anuria and the lifting of the urea nitrogen and blood creatinine. The therapy is very difficult when the HRS appear. The falling off of the glomerular filtration rate and the kidney blood flow has appeared before the lifting of the blood creatinine and urea nitrogen. That is called the subclinical HRS. It can not be identified by the conventional kidney function check. The subclinical HRS will soon develop to be the HRS when it is affected by the endotoxemia, excess diuresis and the bleeding-out of the digestive tract. The research indicates that the HRS kidney damage is functional. The kidney function failure can be avoided when the HRS is identified in early stage and traded properly. So, the early stage diagnosis of the subclinical HRS is very useful to the treatment of the HRS.In this paper, the change of the indexes which are the blood urea nitrogen, creatininer,β2-microglobulin of the blood, 24h albumin of the urine, microalbuminuria of the urine,α1-microglobulin of the urine,β2-microglobulin of the urine, the N-Acetyl-β-glucosaminase of the urine in the different stage of the HRS patients has been detected by the biological method. The interlobar arteria hemodynamics indexes are got by the colored Doppler ultrasonic technology. The correlation between the resistance index of the kidney blood hemodynamics and the laboratory indexes of the blood and urine which have the statics meaning is researched to find the positive correlation of them. The aim of the analysis is to find the sensitive indexes which could reflect the early stage pathologic change of the kidney. It can give more proper, accurate and effective method to the early stage diagnose of the subclinical HRS and the appreciation of the prognosis.The results of the research indicate that the indexes of theα1-microglobulin of the urine,β2-microglobulin of the urine and the N-Acetyl-β-glucosaminase of the urine of the patients that are in the stage of the cirrhosis lose compensatory are more higher when P<0.01. The analysis of the line correlation indicates the coefficient of association between theα1-microglobulin and the N-Acetyl-β-glucosaminase of the urine is most big in the correlation of the biological indexes. It is the positive correlation. The coefficient of association between theα1-microglobulin of the urine and the resistance index of the interlobar arteria hemodynamics is most big in the correlation of the biological indexes and the kidney dynamics. It is the positive correlation. It can be seen theα1-microglobulin of the urine is the most sensitive index for the diagnose of the subclinical HRS...
