Objective:Lung cancer is now the greatest health risks to human, particularly lung adenocarcinoma, not only with a high incidence and poor prognosis after early stage of lymphatic metastasis. Tumor invasion and metastasis is the leading cause of death of cancer, but the reason that tumor invasion and metastasis are still unknown. Lung cancer treatment is not satisfactory, the current 5-year survival rate of surgery only about 30% to 40%. Wwhen the tumor volume more than 2~3mm around, they need to be supplied nutrition and angiogenesis growth factor. In 1997 O'Relly and Folkman found that endostatin, which through inhibition of endothelial cell proliferation and prevent the transfer to inhibit angiogenesis. So to explore the relationship between the expression of endostatin in lung adenocarcinoma and the lymphatic generation and metastasis by detect the expression of endostatin in lung adenocarcinoma and the microlymphatic vessel density (MLVD) is important. This study was designed to test endostatin in lung adenocarcinoma, and explore the relationship between endostatin and lymphangiogenesis,lymph node metastasis,the clinical pathological features of lung adenocarcinoma.Materials and methods:The 54 lung adenocarcinoma specimens come from the thoracic surgery of the second hospital of Shandong University from 2005 to 2007. The patients who receive surgery were confirmed by pathological lung adenocarcinoma. Of which:30 males and 24 females, aged 52 to 69 years old(average age:62.52±6.74 years);poorly differentiated adenocarcinoma in 26 cases,28 cases in moderately and well-differe-ntiated adenocarcinoma;ⅠandⅡperiod are 29 cases,ⅢandⅣperiod are 25 cases. None of the patients received radiation treatment and chemotherapy. Detected by immunohistochemistry, according to staining intensity and density of the tumor cells were divided into four endostatin staining intensity:negative (-), mild (+), moderate (++), Severe (+++)。Defined as negative and mildly negative expression of endostatin, moderate and severe defined as endostatin expression. D2-40 positive staining for the lymphatic endothelial cells account, at low power(X 100) view in the lymphatic concentration "hot spots", at high power(X 200) View the lymphatic count the number of terms for the micro-vessel density (MLVD). Combination of clinicalpathological features of lung adenocarcinoma and analysis the relationship between endostatin expression and the clinicopathological features of lung adenocarcinoma.Results:Endostatin in 6 of 54 cases lung adenocarcinom expression was detected negative (-),11 of 48 cases lung adenocarcinoma weak expression was detected as negative,14 cases positive and 23 cases strong positive expression counted as positive. The Strong positive rate of endostatin expressed in lung adenocarcinoma is 48%, the positive rate of endostatin expressed in lung adenocarcinoma is 24%. In endostatin-positive and strong positive expression of lung cancer, the cell cytoplasm and cell membrane were stained yellow; weak expression in lung adenocarcinoma endostatin, only see a small amount of cytoplasm of infected cells to yellow; strong endostatin expression in lung adenocarcinoma, D2-40 staining was sparse lymphatic vessels, lymphatic vessel density, and low levels of lymphangiogenesis in lymphatic vessels under the microscope the number of statistics to calculate lymphatic density 3.97±0.88; weak endostatin expression in lung adenocarcinoma, was dyed yellow in the tumor tissue and tumor can be seen between the D2-40 positive staining between the tumor and tumor lymphatic vessel density was high, indicating high levels of lymphangiogenesis, lymphatic vessel density was 9.53±1.06, the difference was statistically significant (P<0.01). The expression of endostatin has closely relationship with lung tumor size, P-TNM stage, tumor differentiation, presence of Lymph nodes metastasis and micro-vessel density(P<0.05), but no significant relationship with sex, age(P>0.05).Conclusion:Endostatin express in lung adenocarcinoma and has closely relationship with lymphangiogenesis and lymph node metastasis.
