Objective:To investigate the influence of laparoscopic radical resection of rectal carcinoma with pelvic autonomic nerve preservation on the urinary and sexual functions of male patients with rectal cancer during the comparative study of laparoscopic radical resection (LRR) and open radical resection (ORR),both were pelvic autonomic nerve preservation(PANP).Methods:The data from Jan.2002 to Jan.2010 of 83 patients with rectal cancer were assigned to laparoscopic (45 cases) or open (38 cases) resection, all patients were treated by total mesorectal excision (TME). The urinary functions (urinary catheterization,maximal flow rate,voided volume,residual urine volume) and sexual functions (erectile function and ejaculate function) were assessed.Results:Significant differences were found after surgery between LS and OS in urinary catheterization ((2.5±1.3d vs (4.4±1.1d), maximal flow rate ((19.6±3.2) ml/s vs (28.5±2.4) ml/s), voided volume ((289.7±47.5) ml vs (322.4±37.0) ml), residual urine volume ((12.8±3.4) ml vs (19.1±2.7) ml).The incidence of erectile dysfunction were 6/45(13.58%) and 10/38(25.63%), the incidence of ejaculate dysfunction were 10/45(22.22%) and 12/38(31.58%). the incidence of function dysfunction were 7/45(15.56%) and 8/38(21.05%). The significant differences were doserved between the two groups (P<0.01)Conclusion:Compare with the ORR, The LRR can enhance the urinary and sexual functions of male patients with rectal cancer and improve patients post-operation quality of life, so we can say that it is both feasible and safe and it is absolutely necessary to spread it.
