Background and ObjectiveRectal cancer is one of the most common malignant tumors in the digestive system.With the increasing eating habits and aging of population,the incidence and mortality of rectal cancer in China are increasing.The age of onset is also younger.At present,the treatment of rectal cancer mainly follows the surgical removal of the focus,the cleaning of the surrounding lymph nodes,combined with the concept of chemotherapy and radiotherapy.For middle-low rectal cancer,because of its location and surrounding deep pelvic anatomical complexity increases the difficulty of operation,so the surgery in the case of radical cure will protect the pelvic autonomic nerve damage has a significant impact on patients' quality of life and survival.The aim of this study is to compare the effects of two operative methods of pelvic autonomic nerve preservation and total mesorectal excision on postoperative urination and sexual function under the open abdomen in male patients,so as to provide data support for clinical practice.MethodsThis study is in accordance with the preoperative exclusion in patients with urinary system diseases and sexual dysfunction,collect and select our hospital and August 2015 to August 2016 during the clinical data of surgical treatment of less than or equal to 60 year old men,a total of 56 cases of patients with rectal cancer were analyzed retrospectively,including 28 cases of TME+PANP surgery as experimental group,28 cases of TME surgery as the control group,by comparative analysis of the statistical analysis of patients with postoperative urinary function,sexual function,postoperative one year survival rate and recurrence rate after comparison of two surgical methods for differences between the quality of life of patients after operation and the quality of life for patients in clinical surgery,select a more reasonable way to provide the basis for clinical practice.ResultThe 56 cases of middle and low rectal cancer involved in the study were successfully performed radical resection,and no cases were lost after the operation.Statistical results showed that there was no significant difference in age,tumor distance from Anal border,tumor diameter,TNM stage and pathological type between the two groups(P>0.05),but there was statistical difference in operative time(P<0.05).The preservation rate of postoperative voiding function in group PANP and group TME was 20/28(71.4%)and 10/28(35.7%)respectively,and the preservation rate of erectile function was 20/28(71.4%)and 11/28(39.3%)after operation,and the preservation rate of ejaculation function was 19/28(67.8%)and 11/28(39.3%)after operation.Group PANP has protective effect on postoperative urination and sexual function,and has statistical significance compared with group TME(P<0.05),and does not increase postoperative recurrence rate and survival time.ConclusionDue to the economic and the improvement of living standards,awareness of disease prevention is improved and the related inspection way to upgrade,make the male middle-low rectal cancer patients are younger,for pelvic autonomic nerve preservation for rectal cancer after a significant reduction in urinary function and sexual dysfunction rate at the same time,also does not increase the recurrence rate of 1 years and decrease survival rate of 1 years,it is worthy of clinical publicity and promotion.
