Objective To investigate the changes of the serum apelin level and its mechanism in patients with metabolic syndrome.Methods Sixty-nine patients with metabolic syndrome and Sixty-three subjects with normal glucose regulation (NGR) were selected and the former was divided into metabolic syndrome with T2DM(MS I group) and metabolic syndrome with impaired glucose regulation(MS II group) subgroups according to the levels of blood glucose, and the body mass index(BMI)≥25kg/m2;the latter was divided into overweight/obesity and normal control(NC) subgroups according to body mass index(BMI)≥25kg/m2 or <25kg?m2 and the normal control(NC) subgroups'body mass index(BMI)≥18.5kg?m2. Meanwhile, the height, weight, waist circumference, blood pressure, fasting insulin, fasting plasma glucose (FPG), HbA1C and lipid profile level were also measured, BMI and insulin sensitivity was assessed by HOMA-IR. The level of fasting serum apelin was measured by ELISA .Results①Compared to normal control group, the fasting serum apelin concentration was increased in both MS and overweight/obesity groups(P<0.01). The fasting serum apelin level in MS combined with type 2 diabetes mellitus subgroup was significantly higher than that of impaired glucose regulation MS subgroup(P<0.05) and overweight/obesity group(P<0.01). The fasting serum apelin level in impaired glucose regulation MS subgroup was significantly higher than that of overweight/obesity group(P<0.05). The level of fasting serum apelin,FINS,ln(HOMA-IR),FPG,2hPG,HbA1C,SBP,TC,LDL-C in MS was significantly higher than that of the normal glucose regulation(all P<0.05).②Correlation analysis showed that the serum apelin was positively correlated to Weight,BMI,Waist,SBP,FPG,2hPG,HbA1C,TC,LDL-C,FINS,ln(HOMA-IR) (r=0.187, r= 0.494, r=0.425, r=0.585, r=0.496, r=0.546, r=0.457, r=0.351, r=0.235, r=0.509, r=0.600, all P<0.05).③Partial correlation analysis showed that the serum apelin was also positively correlated to ln(HOMA-IR),FINS,TC(r2=0.211, r2=0.178, r2=0.223, all P<0.05).④In a multiple step-wise regression analysis, ln(HOMA-IR), BMI and TC were independent factors related to apelin (r2=0.544, r2=0.494, r2=0.293, all P<0.05).Conclusions①It maybe the increased insulin resistance which caused the level of serum apelin increased in the patients with MS group.②The increased of serum apelin level is closely related to overweight/obesity, diabetes/impaired glucose regulation, hypertension, dyslipidemia and other metabolic abnormalities of metabolic syndrome.
