Background.Hemodialysis hypotension is the most common complication, about 20-40%. Dialysis hypotension not only directly affect the dialysis results, ultrafiltration, but also the stability of cardiovascular function is affected, even to death. Clinical Practice shows Regular hemodialysis hypotension occurs due to lead the way more. Prevention of dialysis hypotension, reduce the incidence of very great significance.In recent years, reported in the literature at home and abroad have more than one low-temperature dialysis can improve the stability of hemodynamics, decreased blood pressure and related complications, substantially reducing the cost of care measures and medical care, but for the feeling of respect in dialysis patients research is still relatively small.Objectives: The test taken from patients with cross-dialysis method, 20 patients with recurring long-term dialysis patients with low blood pressure with 37℃and 35℃respectively dialysis dialysate for 4 weeks, the last four weeks to enable patients to choose their own temperature dialysis feel better. Records of body temperature during dialysis, heart rate, pulse, blood pressure, temperature, dialysis hypotension were observed incidence of cool dialysate on hemodynamic stability in patients with the role and comfort through the post.Methods: Select long-term hemodialysis in our hospital and 20 patients on dialysis. Repeatedly during dialysis hypotension, accompanied by dizziness, cold sweats, nausea and vomiting, and two months, the incidence of hypotension> 30%. Informed consent to participate in trials, including 9 males, 11 females, aged from 20 to 78 years, mean 45±20 years. All patients with dialysis three times a week, each time 4h. Three phases of the experiment (12 weeks) were taken from patients with cross-dialysis method, were 37℃and 35℃with dialysate dialysis for 4 weeks, the last four weeks to enable patients to choose their own temperature dialysis feel better. The first phase of dialysis at room temperature (37℃), the second phase of cool dialysis (35℃), the third stage with better patient comfort temperature (37℃or 35.0℃) for dialysis.Results:1. There was no change in the post-dialysis axillary temperature with the use of dialysate at 37℃, but it decreased with the use of bath at 35℃(p<0.01). The mean arterial pressure post dialysis was significantly higher in patients dialyzed with cool dialysate compared with those dialyzed with normal dialysate (P<0.05); The BP decreased after dialysis with both cool and normal temperature dialysate(P<0.05) The heart rate increased with bath temperature at 37℃(P<0.05) and was not altered with bath temperature at 35℃..2. Cool dialysis (35℃) in the muscle cramps, nausea, vomiting, headache, sweating and other symptoms appeared significantly lower than the frequency of dialysis at room temperature (37℃) (P <0.05). Low number of dialysis patients chills significantly more than the normal temperature of dialysis (P <0.01). Hemodialysis patients from the previous 4 weeks at room temperature to low temperature dialysis hypotension after dialysis significantly reduced the frequency of symptoms.3. The third stage, 17 (88%) still chose cool dialysis patients, the remaining three patients still prefer to room temperature dialysis by 12%. Most dialysis patients after low temperature was more comfortable than before and full of vitality.Conclusions:1. cool dialysate improves tolerance to HD and is an important factor in stabilizing BP during HD.2. Dialysis in terms of relative temperature, cool dialysis and hemodialysis as maintaining the stability of dialysis to reduce the incidence of hypotension and improve the quality of life of patients3. Cool dialysis is a simple, effective and economical method of dialysis, especially for recurrent patients with symptomatic hypotension. However, not all patients suitable for cool dialysis.
